摘  要:目的研究初发、未服药重度抑郁症(MDD)患者静息态低频振幅率(fALFF)特点,及其与抑郁症状相关性,探讨MDD发病的可能的神经病理机制。方法采用静息态功能磁共振成像(rs-fMRI)计算年龄、性别、受教育年限均匹配的MDD患者(68例)及健康对照组(HC)(72名)的静息态fALFF后进行组间比较,并分析MDD患者异常脑区fALFF值与抑郁症状的相关性。结果初发、未服药MDD患者与HC组相比右侧额中回fALFF值减低,双侧小脑CrusⅠ区、右侧小脑Ⅵ区、右侧颞下回、右侧梭状回、右侧舌回、右侧角回、左侧楔叶fALFF值升高,右侧额中回fALFF改变与抑郁症症状呈正相关,余脑区与抑郁症状之间未发现明显相关性。结论初发、未服药MDD患者右侧额中回、双侧小脑CrusⅠ区、右侧小脑Ⅵ区、右侧颞下回、右侧梭状回、右侧舌回、右侧角回、左侧楔叶存在fALFF值的异常,其中右侧额中回还与抑郁症症状存在明显的正相关,提示这些脑区可能参与初发抑郁症的神经病理过程。Objective To investigate the rs-fMRI fALFF features and their relationship of MDD symptom in first episode drug naive major depressive disorder patients, and discuss the possible psych-neuropathological mechanism. Methods Using rs-fMRI to examine the age, sex and level of education all matched MDD and HC groups, comparing the differences of fALFF between the two groups then analysing the relationship between the difference fALFF values and MDD symptom. Results Compared tothe HC group, MDD group show decreased fALFF in right middle frontal gyrus, and increased fALFF in bilateral cerebellum CrusⅠ,right cerebellumⅥ,right inferior temporal gyrus, right fusiform gyrus, right lingual gyrus, right angular gyrus and left cuneus gyrus, the fALFF signal values of right middle frontal gyrus show significant positive correlation with MDD symptom.We did not find any correlations between the MRI signal values of other brain regions and MDD symptom. Conclusion We found abnormal fALFF in first episode drug naive major depressive disorder patient’s right middle frontal gyrus, right inferior temporal gyrus, right fusiform gyrus, right lingual gyrus, right angular gyrus, left cuneus gyrus, bilateral cerebellum Crus Ⅰ,right cerebellum Ⅵ,and right middle frontal gyrus show significant positive correlation with depression symptom, suggestthat these regions may be involve in the psych-neuropathological process.
