骨盆后环骨折合并腰骶丛损伤的临床研究

摘　　要：目的:研究分析骨盆后环骨折合并腰骶丛损伤。方法:选取从2019年4月-2020年3月,于我院诊治的单侧骨盆后环骨折合并腰骶丛损伤患者60例。所有患者均进行常规急救处理,待其病情稳定后综合临床症状表现、体格检查、影像学检查确定患者病情。此外,以数字法随机分为对照组及观察组各30例,其中对照组患者给予保守治疗,观察组进行减压松懈术治疗,对比2组治疗效果。结果:腓总神经、胫神经以及股神经的感觉与运动障碍均呈逐渐下降趋势;L_(5)、S_(1)感觉支损伤占比显著高于L_(2)、L_(3)、L_(4)而运动支损伤中L_(4)、L_(5)、S_(1)占比均显著高于L_(2)、L_(5)观察组治疗总有效率为83.33%(25/30),显著高于对照组的60.00%(18/30),差异均有统计学意义(均P<0.05)。结论:骨盆后环骨折合并骶丛损伤后感觉与运动障碍主要发生于腓总神经,而神经根感觉支损伤主要集中于L_(5)、S_(1)而运动支损伤主要集中于L_(4)、L_(2)、S_(1)对患者采取保守治疗能达到更加显著的效果。Objective:To study clinical analysis of pelvic ring fractures after merger lumbosacral plexus injury.Methods:Chose from April 2019 to March 2020,in the hospital diagnosis and treatment of unilateral pelvic ring fractures combined 60 cases of lumbosacral plexus injury patients.All patients for regular first aid treatment,for its stable condition after comprehensive clinical symptom,physical examination,imaging studies to determine the patient condition.In addition,in digital method were randomly divided into the conservative group and 30 cases of surgical group.The conservative group patients was given conservative treatment,scores of decompression for the treatment of lax,compared two groups of therapeutic effect.Results:Sural total nerve,tibial nerve and the feeling of femoral nerve and movement disorders were showed a trend of gradual decline;I5 and S1 feel the injury accounts for a significantly higher than the 12,L3 and L4,and movement of L4,L5,S1 accounted in the lesions were significantly higher than I2,L3;Conservative treatment group total effective rate was 83.33%(25/30),which was significantly higher scores of 60.00%(18/30),which were statistically significant differences(all P<0.05).Conclusion:The pelvic ring fractures after merger sacral plexus injury after feeling and movement disorders mainly happens on the phil total nerve,and the damage mainly focused on the L5 nerve root feeling,S1,and exercise the damage mainly focused on L4,L5,S1.For patients to take conservative treatment can achieve more remarkable effect.
