微型外固定架与克氏针对手部复杂关节内骨折患者的疗效及安全性影响

摘　　要：目的:研究微型外固定架与克氏针对手部复杂关节内骨折患者的疗效及安全性影响。方法:选择从2019年3月-2020年3月在我院接受手术治疗的手部复杂关节内骨折患者88例作为观察对象。按随机数表法分成观察组及对照组各44例,对照组常规内固定治疗,观察组则采用微型外固定架与克氏针联合固定,随访6个月;对比2组疗效,手术指标,并发症情况,治疗前后血清炎症因子水平及生活质量变化情况。结果:观察组的优良率是97.73%,较对照组的84.09%明显更高(P<0.05);观察组的术中出血量和住院时间,以及骨折愈合时间分别较对照组的差异均不显著(P>0.05);观察组并发症的总发生率是4.55%,较对照组的20.45%明显更低(P<0.05);观察组治疗后血清白细胞介素-6(IL-6)、白细胞介素-8(IL-8)以及C反应蛋白(CRP)水平分别为(98.47±21.58)ng/L、(78.42±20.09)ng/L、(3,10±0.57)mg/L,均低于对照组的(142.70±32.94)ng/L、(109.24±26.72)ng/L、(5.69±0.78)mg/L(P<0.05);观察组治疗后社会功能、躯体功能、角色功能以及认知功能评分分别为(83.15±4.60)分、(83.75±4.06)分、(84.59±6.22)分、(85.10±4.26)分,均高于对照组的(75.02±4.37)分、(74.19±4.02)分、(79.40±5.87)分、(80.06±3.21)分,差异有统计学意义(P<0.05)。结论:微型外固定架联合克氏针对手部复杂关节内骨折患者的疗效较好,且安全性也较高,值得推广。Objective:To study the effect of micro external fixator and Kirschner on the efficacy and safety of hand complex intraarticu-lar fracture.Methods:From March 2019 to March 2020,88 patients with complicated intra-articular fractures of the hand were selected as observation objects.According to the random number table method divided into observation group and control group 44 cases,Routine internal fixation was given to the control group,and the observation group was fixed by micro external fixator and Kirschner needle,Six months follow-up,the efficacy,surgical indicators,complications,serum inflammatory factors levels and quality of life before and after treatment were compared between the two groups.Results:The excellent rate of observation group was 97.73%,compared with the con-trol group of 84.09%was significantly higher(P<0.05).Intraoperative blood loss and length of stay in the observation group,and the time of fracture healing was not significant compared with that of the control group(P>0.05).The total incidence of complications in the observation group was 4.55%,compared with the control group of 20.45%was significantly lower(P<0.05).The serum levels of inter-leukin-6(IL-6),interleukin-8(IL-8)and C-reactive protein(CRP)in observation group after treatment were(98.47±21.58)ng/L,(78.42±20.09)ng/L,(3.10±0.57)mg/L,respectively.Compared with control group(142.70±32.94)ng/L,(109.24±26.72)ng/L,(5.69±0.78)mg/L(P<0.05).The scores of social function,physical function,role function and cognitive function in the observation group after treatment were(83.15±4.60),(83.75±4.06),(s4.59±6.22),(85.10±4.26),respectively.Com-pared with control group(75.02±4.37)points,(74.19±4.02)points,(79.40±5.87)points,(80.06±3.21)points,the differ-ence was statistically significant(P<0.05).Conclusion:Micro external fixator combined with Kirschner is effective in treating compli-cated intraarticular fracture of hand,and high security,Its worth promoting.
