摘 要:目的探讨儿童维生素A水平与婴幼儿喘息性疾病的相关性,并分析不同病原体所致的喘息维生素A水平之间是否有差异,为临床诊疗提供指导。方法选取2018年12月至2019年12月广东省江门市中心医院儿科诊断为喘息的婴幼儿血液标本250例,采用高效液相色谱法行血液维生素A水平检测。选取同期体检的健康儿童150名,其血维生素A水平作为对照。比较两组血清维生素A水平的差异,并比较维生素A缺乏和婴幼儿喘息性疾病的相关性。同时采集喘息患儿咽拭子,应用多重PCR技术行病毒检测,比较不同病原体所致喘息之间的维生素A水平差异。结果250例喘息患儿中,亚临床维生素A缺乏(SVAD)患儿48例(19.20%),可疑亚临床维生素A缺乏(SSVAD)患儿77例(30.80%),维生素A(VA)正常患儿125例(50.00%)。健康儿童组中,SVAD患儿13例(8.67%),SSVAD患儿28例(18.67%),VA正常患儿109例(72.66%)。喘息患儿组中,病原体阳性率190例(76.00%),其中,病毒感染113例(59.47%),细菌感染26例(13.68%),支原体感染18例(9.47%),细菌病毒混合感染33例(17.37%)。喘息患儿的SVAD及SSVAD发生率均高于健康儿童组(P<0.05),不同病原体之间比较,差异无统计学意义(P>0.05)。结论维生素A的临床缺乏和亚临床缺乏可能与婴幼儿喘息性疾病相关。Objective To explore the correlation between vitamin A(VA)levels and wheezing disease in infants,and to analyze whether there are differences in VA levels among infants with asthmatic disease caused by different pathogens,so as to provide guidance for clinical diagnosis and treatment.Methods Blood specimens were collected from 250 infants diagnosed with wheeze in the department of pediatric internal medicine of Jiangmen Central Hospital in Guangdong Province from December 2018 to December 2019 were selected,and blood VA levels were measured by high performance liquid chromatography.The blood VA levels of 150 healthy children who underwent physical examination at the department of children′s health prevention in Jiangmen Central Hospital in Guangdong Province during the same period were selected as controls.The differences between the two groups in blood VA levels were analyzed and compared,and the correlation between VA deficiency and asthmatic disease in infants was also compared between the two groups.In addition,the multiplex polymerase chain reaction(PCR)technique was applied to detect viruses in pharyngeal swabs of the infants with asthma,and the differences in VA levels among infants with asthma caused by different pathogens were analyzed and compared.Results Among 250 infants with asthma,48 cases(19.20%)infants had subclinical VA deficiency(SVAD),77 cases(30.80%)infants had suspicious subclinical VA deficiency(SSVAD)and 125 cases(50.00%)infants had normal VA.Among the healty chydren group 13(8.67%)children had SVAD,28 cases(18.67%)children had SSVAD,and 109 cases(72.66%)children had normal VA.There were 190 infants had positive pathogens among 250 infants with asthma,with a positivity rate of 76.00%.In all the 190 infants with positive pathogens,113(59.47%)infants suffered from viral infections,26 cases(13.68%)infants suffered from bacterial infections,18 cases(9.47%)infants suffered from mycoplasma infections,and 33 cases(17.37%)infants suffered from mixed bacterial-viral infections.The incidence of SVA
