摘 要:目的通过对国际范围内左束支起搏和希氏束起搏技术的研究进展进行分析,来指导国内相关技术的研究革新。方法使用Web of Science数据库对希氏束起搏和左束支起搏相关的研究文献进行检索,检索时间为1991年1月至2020年12月,检索词为“his bundle pacing”或“left bundle branch pacing”,共得到文献657篇。在进一步筛选中,纳入类型为article和review的638篇文献进行文献计量学分析。结果相关起搏技术的研究于2018年后进入高速发展阶段,近3年来发表197篇研究,约占本研究收录总文献量的30.88%。美国是研究开展最多的国家,发表386篇研究文献,占国际总发文量的60.50%;国内参与该领域研究的人员和合作机构较少。起搏研究的主要方向从最初的解剖结构(his bundle)向技术稳定性(pacing protocol)及远期感知评估(sensing)等热点转变。在希氏束起搏发展的基础上,左束支起搏克服了前者可能潜在的远期阈值不稳定等缺陷,成为新的优化起搏技术。结论左束支起搏和希氏束起搏的研究已进入高速发展的阶段,我国应加大在该领域研究的开展和资源的合理分配,在机制研究的基础上推动大型随机临床试验进行预后和安全性评估。Objective To guide the research and innovation of domestic related technologies by analyzing the research progress of left bundle branch pacing and His bundle pacing technology in the world.Methods The research literature related to his bundle pacing and left bundle branch pacing were searched using Web of Science database.The retrieval time was from January 1991 to December 2020,and the search term was"his bundle pacing"or"left bundle branch pacing".A total of 657 documents were obtained.In further screening,638 articles with the types of article and review were included for bibliometric analysis.Results Research on related pacing technology has entered a rapid development stage after 2018.In the past three years,197 studies had been published,accounting for about 30.88%of the total literature included in this study.The United States is the country with the most research and has published 386 research articles,accounting for 60.50%of the total international publications.There are few domestic personnel and cooperative institutions involved in research in this field.The main direction of pacing research has shifted from the initial anatomical structure(his bundle)to hot spots such as technical stability(pacing protocol)and long-term perception evaluation(sensing).On the basis of the development of his bundle pacing,left bundle branch pacing overcomes the former's possible potential long-term threshold instability and other shortcomings,becoming a new optimized pacing technology.Conclusion The research on left bundle branch pacing and his bundle pacing has entered a stage of rapid development.Our country should increase the development of research in this field and the reasonable allocation of resources,and promote large-scale randomized clinical trials and safety assessment on the basis of mechanism research for prognosis.
