摘  要:目的探讨集束化团队营养干预对结直肠癌化疗患者营养状态及生活质量的影响。方法回顾性研究2018年3月至2020年3月在台州市第一人民医院接受化疗治疗的124例结直肠癌患者临床资料。根据干预方法不同分为对照组和观察组,每组各62例。对照组采用常规化疗饮食护理,观察组采用集束化团队营养干预护理。比较两组出入院时白蛋白(ALB)、前白蛋白(PA)、血红蛋白(Hb)、总胆固醇(TC)和三酰甘油(TG)水平、上臂围、利手握力及三头肌皮褶厚度、体质量指数(BMI)及NRS-2002量表和癌症生活质量问卷-C30(QLQ-C30)评分。结果出院时观察组PA、ALB、Hb、TC和TG水平分别为(223.54±40.79)g/L,(42.67±4.22)g/L,(118.96±8.84)g/L,(2.78±0.66)mmol/L,(0.97±0.44)mmol/L,BMI为(23.34±1.15)kg/m^(2),NRS-2002量表评分为(2.78±0.94)分,与对照组比较,差异有统计学意义(P<0.05);两组患者上臂围、利手握力及三头肌皮褶厚度治疗前后比较,差异无统计学意义(P>0.05);出院时观察组QLQ-C30各维度评分高于对照组(P<0.05)。结论集束化团队营养干预可有效改善结直肠癌化疗患者营养状态,提高生活质量,具有临床应用价值。Objective To explore the effect of bundle nutrition intervention on nutritional status and quality of life of colorectal cancer patients undergoing chemotherapy.Methods The clinical data of 124 patients with colorectal cancer who received chemotherapy in Taizhou First People′s Hospital from March 2018 to March 2020 were retrospectively studied.According to different intervention methods,they were divided into the control group and the observation group,62 cases in each group.The control group received routine chemotherapy diet nursing,while the observation group received bundle nutrition intervention nursing.The levels of albumin(ALB),prealbumin(PA),hemoglobin(Hb),total cholesterol(TC)and triglyceride(TG),upper arm circumference,handgrip strength and triceps skinfold thickness,body mass index(BMI),NRS-2002 and QLQ-C30 scores were compared between the two groups at admission and discharge.Results The levels of PA,ALB,Hb,TC and TG in the observation group when discharged were(223.54±40.79)g/L,(42.67±4.22)g/L,(118.96±8.84)g/L,(2.78±0.66)mmol/L,(0.97±0.44)mmol/L,respectively,BMI was(23.34±1.15)kg/m^(2),and NRS-2002 score was(2.78±0.94)points,with statistical differences compared with the control group(P<0.05).There were no significant differences in upper arm circumference,handgrip strength and triceps skinfold thickness between the two groups before and after treatment(P>0.05).When discharged,the scores of QLQ-C30 in the observation group were higher than those in the control group(P<0.05).Conclusion Bundle nutrition intervention can effectively improve the nutritional status and quality of life of colorectal cancer patients undergoing chemotherapy,and has clinical application value.
