摘　　要：目的探讨西咪替丁联合云南白药鼻饲治疗对新生儿消化道出血的影响。方法选取2016年1月至2020年1月丰城市人民医院收治的54例消化道出血新生儿为研究对象,根据治疗方法的不同分为对照组(云南白药治疗,n=28)和观察组(西咪替丁+云南白药鼻饲治疗,n=26),比较两组患者临床有效率,止血时间、输血量、临床症状改善时间和不良反应发生率。结果观察组临床总有效率为96.15%,高于对照组的64.29%,差异有统计学意义(P<0.05);观察组止血时间(36.22±3.19)h、输血量(314.56±20.45)ml,均低于对照组的(40.58±3.40)h、(338.62±20.63)ml,差异有统计学意义(P<0.001);观察组大便隐血实验转阴(4.33±1.02)d、大便转黄(3.12±0.46)d、胃管引流无血性液体时间(1.66±0.33)d均短于对照组的(5.93±1.36)d、(4.02±0.96)d、(2.06±0.48)d,差异有统计学意义(P<0.001);观察组不良反应总发生率为11.54%,略低于对照组的14.29%,差异无统计学意义(P>0.05)。结论西咪替丁联合云南白药鼻饲治疗新生儿消化道出血的临床效果理想,可有效缩短止血时间,降低输血量,缩短临床症状改善时间。Objective To explore the effect of cimetidine combined with Yunnan Baiyao nasogastric treatment on neonatal gastrointestinal bleeding.Methods Fifty-four neonates with gastrointestinal hemorrhage who were admitted to Fengcheng People′s Hospital from January 2016 to January 2020 were selected as the research objects.According to the different treatment methods,they were divided into the control group(Yunnan Baiyao treatment,n=28)and the observation group(cimetidine+Yunnan Baiyao nasogastric treatment,n=26).The clinical effective rates,hemostasis time,amounts of blood transfusion,clinical symptom relief time and incidences of adverse reactions of the two groups were compared.Results The total effective rate of the observation group was 96.15%,which was higher than 64.29%of the control group,and the difference was statistically significant(P<0.05).The hemostasis time(36.22±3.19)h and amount of blood transfusion(314.56±20.45)ml in the observation group were lower than(40.58±3.40)h and(338.62±20.63)ml in the control group,and the differences were statistically significant(P<0.001).In the observation group,the time of stool occulting blood test turning negative(4.33±1.02)d,stool turning yellow(3.12±0.46)d,and no bleeding fluid in gastric tube drainage(1.66±0.33)d were shorter than(5.93±1.36)d,(4.02±0.96)d and(2.06±0.48)d of the control group,and the differences were statistically significant(P<0.001).The total incidence of adverse reactions in the observation group was 11.54%,which was slightly lower than 14.29%in the control group,but the difference was not statistically significant(P>0.05).Conclusion The clinical effect of cimetidine combined with Yunnan Baiyao?nasogastric treatment on neonatal gastrointestinal bleeding is ideal,which can effectively shorten the hemostasis time,reduce the amount of blood transfusion,and shorten the clinical symptom relief time.
