摘 要:目的探讨超声造影在乳腺积乳囊肿中的应用。方法选取2019年4月至2020年4月湖北省中医院乳腺积乳囊肿患者124例,均行常规超声和超声造影诊断,以病理结果为“金标准”,统计常规超声和超声造影诊断乳腺积乳囊肿结果,对比两者诊断乳腺积乳囊肿的诊断效能。结果124例乳腺积乳囊肿患者,病理切缘结果确诊阳性60例,阴性64例;采用常规超声诊断出真阳性45例,真阴性36例;采用超声造影诊断出真阳性56例,真阴性47例;超声造影诊断灵敏度93.33%(56/60)、特异度73.44%(47/64)、准确度83.06%(103/124),高于常规超声诊断的75.00%(45/60)、56.25%(36/64)、65.32%(81/124);漏诊率3.23%(4/124),低于常规超声诊断的12.10%(15/124)(P<0.05);超声造影阳性预测值76.71%(56/73)、阴性预测值92.16%(47/51),较常规超声的61.64%(45/73)、70.59%(36/51)高(P<0.05);增强强度、增强后病灶是否增大、穿支血管、周围放射状增强及显影时间比较,差异有统计学意义(P<0.05)。结论超声造影诊断乳腺积乳囊肿,可提高诊断灵敏度、特异度、准确度,降低漏诊率,应用价值较高,对指导临床治疗采取针对性干预措施具有积极意义。Objective To explore the application of contrast-enhanced ultrasound in galactocele.Methods A total of 124 patients with galactocele in Hubei Provincial Hospital of Traditional Chinese Medicine from April 2019 to April 2020 were selected and diagnosed with conventional ultrasound and contrast-enhanced ultrasound.With the pathological results as the“gold standard”,the results of conventional ultrasound and contrast-enhanced ultrasound in the diagnosis of galactocele were statistically analyzed and the diagnostic efficacy of the two methods in the galactocele was compared.Results Among 124 cases of galactocele,60 cases were confirmed as positive and 64 cases as negative for pathological margin.Forty-five cases were diagnosed as true positive and 36 cases as true negative by conventional ultrasound.There were 56 cases of true positivity and 47 cases of true negativity in the diagnosis by contrast-enhanced ultrasound.The sensitivity,specificity and accuracy of contrast-enhanced ultrasound were 93.33%(56/60),73.44%(47/64)and 83.06%(103/124),respectively,higher than of conventional ultrasound in the diagnosis(75.00%[45/60],56.25%[36/64]and 65.32%[81/124],respectively).The missed diagnosis rate was 3.23%(4/124)lower than 12.10%(15/124)of conventional ultrasound in(P<0.05).The positive predictive value and negative predictive value of contrast-enhanced ultrasound were 76.71%(56/73)and 92.16%(47/51),respectively,higher than(61.64%[45/73]and 70.59%[36/51])of conventional ultrasound(P<0.05).There were significant differences in enhancement intensity,whether the lesion was enlarged after enhancement,the perforator arteries,the peripheral radial enhancement,and developing time(P<0.05).Conclusion Contrast-enhanced ultrasound can improve the diagnostic sensitivity,specificity and accuracy and reduce the rate of missed diagnosis in the diagnosis of galactocele,with high application value.It is of positive significance for the clinical treatment and targeted interventions.
