摘　　要：目的探讨血清可溶性B7-H3(sB7-H3)在原发性胆汁性胆管炎(PBC)患者中的表达水平和临床预后评价。方法选取初诊诊断为PBC的患者37例、体检健康者50例,并收集基本临床资料。用ELISA法测定血清sB7-H3及白细胞介素6(IL-6)的浓度;用全自动生化分析仪测定丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、碱性磷酸酶(ALP)、γ-谷氨酰基转移酶(GGT)、总胆红素(T-Bil)等指标,同时用多重微珠流式荧光法和免疫印迹法测定自身抗体。对sB7-H3、IL-6与肝功能各项指标进行Spearman相关性分析。结果PBC组血清sB7-H3以及IL-6较对照组均显著升高,差异有统计学意义(P<0.0001以及P=0.0055);Mayo评分高的患者sB7-H3水平显著高于Mayo评分低的患者(P=0.0006);anti-GP210阳性PBC患者的sB7-H3水平较阴性组显著升高,差异有统计学意义(P=0.0015);Spearman相关性分析发现sB7-H3、IL-6血清浓度均与ALT、AST、ALP、T-Bil呈正相关关系(P<0.05)。结论sB7-H3在PBC患者血清中显著升高,在PBC患者预后评估中具有潜在的应用价值。Objective To investigate the expression levels of soluble B7-H3(sB7-H3)in serum of patients with primary biliary cholangitis(PBC)and its clinical significance.Methods The basic clinical data from 37 patients with PBC and 50 healthy controls were collected.The levels of serum sB7-H3 and interleukin 6(IL-6)were detected by ELISA.The indexes such as alanine aminotrans-ferase(ALT),aspartate aminotransferase(AST),alkaline phosphatase(ALP),γ-glutamyltransferase(GGT)and total bilirubin(T-Bil)were detected by an automatic biochemical analyzer.Meanwhile,autoantibodies were determined by the multiple bead flow cy-tometry and immunoblotting.The correlations of sB7-H3 with IL-6 and liver function indexes were analyzed by Spearman correlation a-nalysis.Results Serum sB7-H3 and IL-6 levels in PBC patients were significantly higher than that in healthy controls(P<0.0001 and P=0.0055).The levels of sB7-H3 in PBC patients with high Mayo score were significantly higher than that with low Mayo score(P=0.0006).The levels of sB7-H3 in PBC patients with positive anti-GP210 antibodies were significantly higher than that with negative antibodies(P=0.0015).Spearman correlation analysis showed that serum sB7-H3 and IL-6 levels were positively correlated with ser-um ALT,AST,ALP and T-Bil levels(P<0.05).Conclusion Serum sB7-H3 levels in PBC patients increase significantly,indicating that it has potential application value in the prognosis evaluation of PBC patients.
