子宫局部压迫缝合术联合子宫动脉上行支结扎术治疗前置胎盘产后出血的效果观察

摘　　要：目的探讨子宫局部压迫缝合术联合子宫动脉上行支结扎术应用在前置胎盘产后出血中的治疗效果。方法选取中山市博爱医院2018年1月-2020年10月收治的前置胎盘行剖宫产产妇100例,随机分为对照组和观察组各50例。对照组采取常规横行捆绑方式止血,观察组采取子宫局部压迫缝合术联合子宫动脉上行支结扎术,对比2组止血效果、术中术后出血量、并发症发生率。结果观察组产妇的止血总有效率为94.0%,明显高于对照组84.0%(P<0.05);观察组术中、术后2h、12 h、24h出血量均少于对照组(P<0.05);观察组贫血、感染、发热、切口裂开等并发症E总发生率(6.0%)低于对照组(18.0%)(P<0.05)。结论子宫局部压迫缝合术联合子宫动脉上行支结扎术应用在前置胎盘产后出血的治疗当中,能够快速止血,降低术中术后出血量,减少并发症的发生,值得临床推广。Objective To investigate the effect of uterine local compression suture combined with uterine artery ascending branch ligation in the treatment of postpartum hemorrhage of placenta previa,method.Methods 100 patients with placenta previa who underwent cesarean section in our hospital from January 2018 to October 2020 were selected and randomly divided into the control group and the observation group with 50 cases in each.The control group was given routine transverse binding hemostasis,while the observation group was given local uterine compression suture combined with uterine artery ascending branch ligation.The hemostatic effect,intraoperative and postoperative bleeding volume and complication rate of the two groups were compared.Results The total effective rate of hemostasis in the observation group was 94.0%,which was significantly higher than that in the control group(84.0%)(P<0.05);the amount of bleeding duri ng operation,2 h,12 h and 24 h after operation was less than that in the control group(P<0.05);the total incidence of anemia,infection,fever,incision dehiscence complications(6.0%)was lower than that of the control group(18.0%)(P<0.05).Conclusion Local uterine compression suture combined with ligation of ascending branch of uterine artery in the treatment of postpartum hemorrhage of placenta previa can quickly stop bleeding,reduce the amount of intraoperative and postoperative bleeding,and reduce the occurrence of complications,which is worthy of clinical promotion.
