摘　　要：膀胱疼痛综合征/间质性膀胱炎(BPS/IC)是常见的下尿路疾病之一。其中,慢性疼痛作为困扰患者最严重的症状,其发病机制可分为解剖结构的改变、尿液菌群的改变、尿液中有害因子的增加及炎症细胞、因子活化等膀胱内因素,以及低雌激素水平及阴道环境的改变、精神状态的改变、免疫源性炎症、神经源性炎症或合并其他疾病等膀胱外因素。目前主要治疗策略为修复患者缺损的膀胱上皮并恢复膀胱神经元功能以及行为及饮食治疗等全身治疗,缓解患者症状。未来可通过对BPS/IC疼痛具体发生机制的进一步探索和研究进行病因治疗,从而提高BPS/IC的治疗效果。Bladder pain syndrome/interstitial cystitis(BPS/IC)is one of the common lower urinary tract disorders.The pathogenesis of chronic pain,one of the most serious symptoms,can be divided into intra-vesical factors such as altered anatomy,altered urinary flora,increased harmful factors in the urine and activation of inflammatory cells and factors,and extra-vesical factors such as low estrogen levels and altered vaginal environment,altered mental status,immunogenic inflammation,neurogenic inflammation or other co-morbidities.The treatment strategies currently focus on repairing the patient′s defective bladder epithelium and restoring bladder neuron function as well as systemic therapy including behavioral and dietary therapy to relieve the patients′symptoms.The treatment outcomes for BPS/IC can be improved by etiological treatment through further exploration and research of the specific mechanisms of BPS/IC pain.
