骨搬运技术在老年胫骨开放性骨折后感染性骨不连患者中应用效果

摘　　要：目的研究分析骨搬运技术在老年胫骨开放性骨折后感染性骨不连患者中的应用效果及对血液流变学指标的影响。方法选取2018年5月至2020年1月安阳市人民医院收治的老年胫骨开放性骨折后感染性骨不连患者94例进行回顾性分析。按照治疗方法分组,每组47例,观察组行骨搬运技术治疗,对照组实施传统方式治疗,观察两组患者疗效。结果观察组患者在治疗后其手术时间(93.66±8.74)min、住院时间(14.25±5.78)d、疼痛视觉模拟评分(VAS)(1.36±0.10)分、Fugl-Meyer评测法(FMA)评分(29.43±1.35)分,对照组手术时间(101.23±8.28)min、住院时间(17.49±5.63)d、VAS评分(1.87±0.56)分、FMA评分(26.59±2.28)分,组间对比,差异有统计学意义,P<0.05。观察组患者的并发症发生率4.26%低于对照组17.02%,P<0.05。观察组治愈28人、有效14人、无效5人,对照组治愈18人、有效21人、无效8人,组间比较,差异有统计学意义,P<0.05。术后1个月观察组患者血浆黏度(1.51±0.09)mPa·s、红细胞聚集指数(2.03±0.28)、高切全血黏度(4.66±0.38)mPa·s、低切全血黏度(10.26±2.87)mPa·s均显著低于对照组的血浆黏度(1.73±0.11)mPa·s、红细胞聚集指数(2.29±0.23)、高切全血黏度(5.97±0.32)mPa·s、低切全血黏度(11.55±2.52)mPa·s,P<0.05。结论骨搬运技术治疗老年胫骨开放性骨折后感染性骨不连具有显著疗效,可有效改善患者血液流变学指标,有效促进患者骨折愈合。Objective To study and analyze the application effect of bone handling technology in elderly patients with infectious nonunion after open tibial fracture and its effect on hemorheological indexes.Me thods 94 elderly patients with infectious nonunion after open tibial fracture treated in Anyang People’s Hospital from May 2018 to January 2020 were selected for retrospective analysis.They were grouped according to the treatment methods,with 47 cases in each group.The observation group was treated with bone handling technology,and the control group was treated with traditional methods.The curative effects of the two groups were observed.Re s ults After treatment,the observation group had the operation time(93.66±8.74)min,hospital stay(14.25±5.78)d,pain visual analogue score(VAS)(1.36±0.10),Fugl-Meyer assessment(FMA)score(29.43±1.35),and the control group had the operation time(101.23±8.28)min,hospital stay(17.49±5.63)d,pain visual analogue score(VAS)(1.87±0.56),Fugl-Meyer assessment(FMA)score(26.59±2.28),and there was significant difference between the groups(P<0.05).The incidence of complications in the observation group was 4.26%,lower than 17.02%in the control group(P<0.05).There were 28 cured,14 effective and 5 ineffective in the observation group and 18 cured,21 effective and 8 ineffective in the control group.There was significant difference between the groups(P<0.05).One month after operation,the plasma viscosity(1.51±0.09)mPa·s,erythrocyte aggregation index(2.03±0.28),high shear w hole blood viscosity(4.66±0.38)m Pa·s,low she ar whole blood viscosity(10.26±2.87)mPa·s in the observation group were significantly lower than those in the control group with the plasma viscosity(1.73±0.11)mPa·s,erythrocyte aggregation index(2.29±0.23),high shear whole blood viscosity(5.97±0.32)mPa·s and low shear whole blood viscosity(11.55±2.52)mPa·s,P<0.05.Conclus ion Bone handling technology has a significant effect in the treatment of infectious nonunion after open tibial fracture in the elderly.It can e
