玉红生肌膏治疗糖尿病足溃疡的临床研究

摘　　要：目的:观察玉红生肌膏治疗糖尿病足溃疡湿热毒盛证的临床疗效。方法:将60例糖尿病足溃疡湿热毒盛证患者随机分为治疗组和对照组各30例。两组患者均予糖尿病饮食指导、胰岛素控制血糖、改善微循环、营养神经、控制感染等综合治疗,治疗组患者予生肌玉红膏外敷,对照组患者予湿润烧伤膏外敷,每日换药1次,治疗4周。记录两组患者溃疡面积、溃疡量化积分,计算愈合率,并评价临床疗效。结果:治疗组患者临床疗效优于对照组,差异有统计学意义(P<0.05);治疗组愈合率为73.33%(22/30),对照组愈合率为50.00%(15/30),两组愈合率比较,差异无统计学意义(P>0.05);治疗组患者溃疡愈合时间短于对照组,差异有统计学意义(P<0.05);两组患者治疗后溃疡面积和溃疡量化评分均下降(P<0.05),且治疗组患者治疗后溃疡面积和溃疡量化评分均低于对照组(P<0.05)。结论:玉红生肌膏治疗糖尿病足溃疡湿热毒盛证临床疗效确切,能有效提高糖尿病足溃疡治愈率,促进创面肉芽组织形成,减少局部渗出,减轻创面水肿、疼痛,促进创面愈合,缩短治疗时间。Objective: To observe the effect of Yuhong Shengji Plaster in treating diabetic foot ulcer with damp-heat and toxin syndrome. Methods: Totally 60 patients with diabetic foot ulcer with damp-heat and toxin syndrome were randomly divided into treatment and control groups, 30 cases in each group. Patients were given comprehensive treatment of diabetes diet guidance, insulin control of blood sugar, improvement of microcirculation,nutrition nerve and control of infection in two groups. Patients were treated with Yuhong Shengji Plaster in treatment group and moist exposed burn ointment(MEBO) in control group, once a day. The course of treatment was four weeks. The ulcer area and ulcer quantitative score were recorded and the healing rate was calculated in the two groups. The clinical efficacy was evaluated. Results: The treatment group showed better curative effect than control group, and the difference was statistically significant(P<0.05). The healing rate was 73.33%(22/30) in treatment group and 50.00%(15/30) in control group. There was no significant difference in healing rate between the two groups(P>0.05). The treatment group shoed shorter ulcer healing time than control group,and the difference was statistically significant(P<0.05). The ulcer area and quantitative score decreased in the two groups after treatment(P<0.05), and the treatment group showed lower ulcer area and quantitative score than control group after treatment(P<0.05). Conclusion: Yuhong Shengji Plaster is effective in the treatment of diabetic foot ulcer, and it can effectively improve the cure rate of diabetic foot ulcers, promote granulation tissue growth,reduce local exudation, reduce wound edema and pain, promote wound healing, and shorten the treatment time.
