袢钢板弹性悬吊治疗下尺桡关节脱位的临床疗效

摘　　要：目的探讨袢钢板弹性悬吊下尺桡关节(DRUJ)治疗DRUJ脱位的临床疗效。方法选取于2016年1月至2020年6月于郑州大学第一附属医院收治的确诊为DRUJ脱位的患者15例,随访时间6~18个月,对比其术前术后腕关节评分Gartland-Werley腕关节评分、术后患侧及健侧腕关节功能、术后并发症。术后两侧关节功能比较采用配对t检验。结果随访期间所选患者无切口感染、脱位复发等并发症发生。术后复查X线片示关节脱位得到有效复位且合并患侧尺桡骨远端骨折者骨折均已良好愈合,末次随访时根据Gartland-Werley评分标准评价腕关节功能:优10分,良4分,可1分。术后腕关节主动活动度健侧与患侧比较差异无统计学意义[屈伸活动度为,(109.33±15.91)°比(106.40±16.20)°,t=1.596,P>0.05;尺桡偏活动度为,(61.33±6.67)°比(59.87±7.69)°,t=1.703,P>0.05;旋前旋后活动度为,(111.67±11.44)°比(109.13±13.64)°,t=1.557,P>0.05]。结论带袢钢板弹性悬吊治疗DRUJ脱位患者术后腕关节功能恢复良好,随访疗效满意。Objective To investigate the clinical efficacy of loop plate elastic fixation in the treatment of distal radioulnar joint dislocation.Methods Totally,15 patients with distal radioulnar joint dislocation admitted to the First Affiliated Hospital of Zhengzhou University from January 2016 to June 2020 were selected,and followed up for 6 to 18 months.The preoperative and postoperative wrist score,Gartlan-Werley wrist score,postoperative wrist function on the affected side and healthy side,and postoperative complications were analyzed.Postoperative bilateral joint function was compared by paired t test.Results No complications such as incision infection and dislocation recurrence occurred in all patients during the follow-up period.Postoperative radiographs showed an effective reduction of joint dislocation and good healing in all patients with distal ulnar and radius fractures on the affected side.At the last follow-up,wrist joint function was evaluated according to the Gartland-Werley wrist score:excellent,10 cases;good,4 cases;possible:1 case.There was no significant difference in the active range of motion between the healthy wrist and the affected wrist[flexion and extension range of motion:(109.33±15.91)°vs.(106.40±16.20)°,t=1.596,P>0.05;Ulnar radial deviation range of motion:(61.33±6.67)°vs.(59.87±7.69)°,t=1.703,P>0.05;pronation and supination range of motion:(111.67±11.44)°vs.(109.13±13.64)°,t=1.557,P>0.05].Conclusion The wrist function of patients with the suspension treatment of the distal ruler joint dislocation was well restored after surgery,and the follow-up effect was satisfactory.
