股骨近端防旋髓内钉内固定术治疗股骨粗隆间骨折患者的效果

摘　　要：目的:观察股骨近端防旋髓内钉(PFNA)内固定术治疗股骨粗隆间骨折患者的效果。方法:选取60例股骨粗隆间骨折患者为研究对象,按照随机数字表法分为对照组和观察组各30例。对照组采用股骨近端解剖型锁定钢板(LCP)内固定术治疗,观察组采用PFNA内固定术治疗,比较两组治疗后髋关节功能优良率、生命质量[健康状况调查简表(SF-36)]评分、并发症发生率和手术相关指标水平。结果:观察组髋关节功能优良率为80.00%,高于对照组的53.33%,差异有统计学意义(P<0.05);观察组生理机能、生理职能、躯体疼痛、一般健康状况、精力、社会功能、情感职能和精神健康等生命质量评分均高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为6.67%,低于对照组的40.00%,差异有统计学意义(P<0.05);观察组手术时间、住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。结论:PFNA内固定术治疗股骨粗隆间骨折患者可提高髋关节功能优良率和生命质量评分,减少术中出血量,缩短手术时间及住院时间,降低并发症发生率,其治疗效果优于LCP内固定术。Objective:To observe effects of proximal femoral anti-rotation intramedullary nail fixation(PFNA)in treatment of femoral intertrochanteric fractures.Methods:60 patients with femoral intertrochanteric fractures were selected as the research objects,and were divided into control group and observation group according to the random number table method,30 cases in each group.The control group was treated with proximal femoral anatomical locking plate fixation(LCP),while the observation group was treated with PFNA fixation.After the treatment,the excellent and good rate of hip joint function,the quality of life[short form health status survey scale(SF-36)]score,the complication rate and the operation-related index levels were compared between the two groups.Results:The excellent and good rate of hip joint function in the observation group was 80.00%,which was higher than 53.33%in the control group,and the difference was statistically significant(P<0.05).The scores of quality of life such as physiological function,physiological role,somatic pain,general health,energy,social function,emotional function and mental health in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the observation group was 6.67%,which was lower than 40.00%in the control group,and the difference was statistically significant(P<0.05).Further,the operation time and the hospitalization time in the observation group were shorter than those in the control group;the intraoperative blood loss was less than that in the control group;the differences were statistically significant(P<0.05).Conclusions:PFNA fixation in the treatment of the patients with intertrochanteric fractures can improve the excellent and good rate of hip joint function and the quality of life score,reduce the intraoperative blood loss,shorten the operation time and the hospitalization time,and reduce the incidence of complications.Moreover,it is superior to the LCP fixation.
