摘  要:目的探讨320排CT采用自动管电流(CARE Dose 4D)技术联合低剂量对比剂在头颈部血管成像扫描中应用价值。方法选择2018年1月至2019年6月进行CT头颈部血管成像检查的40例患者,随机分为对照组和观察组,每组20例。对照组患者设置管电流120 mA,管电压120 kV,对比剂用量为0.7 ml/kg;观察组患者采用CARE Dose 4D技术参考管电流120 mA,管电压80 kV,对比剂用量为0.5 ml/kg;2组患者均测量双侧颈总动脉、双侧颈内动脉及双侧大脑中动脉M1段,比较2组患者不同位置图像质量的CT值、信噪比(SNR)及有效辐射剂量(ED)。结果观察组患者双侧总动脉、双侧内动脉及双侧大脑中动脉M1段的CT值均明显小于对照组(P<0.05);但2组患者各血管的SNR值之间差异无统计学意义(P>0.05)。两位医师对动脉图像质量评分的一致性较高(Kappa=0.773,P<0.05);2组患者图像后处理及图像整体质量评分均差异无统计学意义(P>0.05)。观察组患者的CTDIvol、DLP及ED均明显低于对照组(P<0.05)。结论320排CT头颈部血管成像中应用CARE Dose 4D技术联合低剂量对比剂扫描方案能够在保证图像质量的同时,降低患者辐射剂量和对比剂危害。Objective To explore the application value of 320-slice CT angiography of the head and neck using combined application reduce exposure(CARE)Dose 4D technology combined with low-dose contrast agent.Methods A total of 40 patients who underwent head and neck CT angiography in our hospital from January 2018 to June 2019 were selected,and randomly divided into the control group(n=200)and the observation group(n=20).In the control group,the tube current was 120mA,the tube voltage was 120kV,and the contrast agent dosage was 0.7ml/kg.The observation group was treated with CARE Dose 4D technology.The reference tube current was 120mA,the tube voltage was 80kV,and the contrast agent dosage was 0.5ml/kg.M1 segment of bilateral common carotid artery,bilateral internal carotid artery and bilateral middle cerebral artery of both groups was measured.The CT value and signal-to-noise ratio(SNR)of image quality at different positions as well as effective radiation dose(ED)were compared in the two groups.Results The CT values of M1 segment of bilateral common artery,bilateral internal artery,and bilateral middle cerebral artery of the observation group were significantly lower than those of the control group(P<0.05);however,there was no statistical difference between the SNR values of the blood vessels in the two groups(P>0.05).The two doctors had relatively high consistency in scoring arterial image quality(Kappa=0.773,P<0.05),and there was no significant difference in image post-processing and overall image quality scores between the two groups(P>0.05).The CTDIvol,DLP and ED of the observation group were significantly lower than those of the control group(P<0.05)Conclusion The application of 320-slice CT angiography of the head and neck using CARE Dose 4D technology combined with low-dose contrast agent can reduce the patient’s radiation dose and hazards of contrast agent as well as ensuring image quality.
