摘　　要：目的:探讨急性胆囊炎伴胆囊结石行单孔腹腔镜胆囊切除术手术时机的选择。方法:回顾分析2020年1月至2021年1月为126例急性胆囊炎伴胆囊结石患者经脐入路行单孔腹腔镜胆囊切除术的临床资料,根据发病时间分为两组,其中64例发病48 h内(A组),62例发病48~72 h(B组),对比分析两组手术时间、术中出血量、术后住院时间及术后并发症等情况。结果:手术均获成功,A组手术时间、术中出血量及术后住院时间均少于B组,差异有统计学意义(P<0.05);两组术后并发症发生率差异无统计学意义(P>0.05)。结论:急性胆囊炎伴胆囊结石于发病48 h内经脐入路行单孔腹腔镜胆囊切除术安全性高、创伤小,而且术后康复快,美容效果好。Objective:To explore the timing choice of single-port laparoscopic cholecystectomy for acute cholecystitis.Methods:The clinical data of 126 patients with acute cholecystitis and cholecystolithiasis who underwent transumbilical single-port laparoscopic cholecystectomy from Jan.2020 to Jan.2021 were retrospectively analyzed.According to the time of onset,they were divided into two groups,64 cases were within 48 h(group A)and 62 cases were within 48-72 h(group B).The operation time,intraoperative blood loss,postoperative hospital stay and postoperative complications were compared between the two groups.Results:All the operations were successful.The operation time,intraoperative blood loss and postoperative hospital stay in group A were significantly less than those in group B(P<0.05),and there was no significant difference in postoperative complications between the two groups(P>0.05).Conclusions:Transumbilical single-port laparoscopic cholecystectomy for acute cholecystitis with gallstones within 48 h of onset has the advantages of high safety,few trauma,fast postoperative recovery and good cosmetic effect.
