超声与超声引导下粗针穿刺活检对乳腺肿块的早期诊断与鉴别价值

摘　　要：目的分析超声与超声引导下粗针穿刺活检对乳腺肿块的早期诊断与鉴别价值。方法选取2020年5月-2021年5月在我院诊治的62例乳腺肿块患者为研究对象,均进行超声检查、超声引导下粗针穿刺活检,分析超声引导穿刺成功率,比较超声检查和超声引导下粗针穿刺活检良恶性诊断率、敏感度、特异度,以及良恶性肿块超声征象特点。结果超声引导下粗针穿刺活检成功率98.39%;乳腺恶性肿块形态不规则、边缘毛糙、边界无回声、内部回声粗杂不均匀、声衰明显、侧后声影不明显、纵横比>0.7发生率均高于良性肿块,差异有统计学意义(P<0.05);超声引导下粗针穿刺活检诊断良性、恶性肿块准确率均高于超声检查,差异有统计学意义(P<0.05),与术后病理结果比较,差异无统计学意义(P>0.05);超声引导下粗针穿刺活检诊断乳腺肿块早期敏感度、特异度均高于超声检查,差异有统计学意义(P<0.05)。结论超声引导下粗针穿刺活检与术后病理诊断结果吻合度较高,可有效提高诊断敏感度和特异度。同时穿刺成功率高,具有良好的应用安全性。在乳腺肿块早期的诊断与鉴别中具有重要的价值。Objective To analyze the early diagnosis and differential value of ultrasound and ultrasound guided needle biopsy for breast masses.Methods A total of 62 patients with breast masses diagnosed and treated in our hospital from May 2020 to May 2021 were selected as the research objects.Ultrasound examination and ultrasound-guided needle biopsy were performed.The success rate of ultrasound-guided puncture was analyzed.The diagnostic rate,sensitivity,specificity,and ultrasonic signs of benign and malignant masses were compared between ultrasound examination and ultrasound-guided needle biopsy.Results The success rate of ultrasound-guided needle biopsy was 98.39 %.The incidence of breast malignant masses with irregular shape,rough edge,no echo boundary,coarse and uneven internal echo,obvious acoustic attenuation,no obvious posterior acoustic shadow and aspect ratio>0.7 was higher than that of benign masses,and the difference was statistically significant( P<0.05).The accuracy of ultrasound-guided needle biopsy in the diagnosis of benign and malignant tumors was higher than that of ultrasound examination,and the difference was statistically significant(P<0.05);while there was no significant difference between the results and postoperative pathological results( P>0.05).The early sensitivity and specificity of ultrasound-guided needle biopsy in the diagnosis of breast masses were higher than those of ultrasound examination,and the difference was statistically significant(P <0.05).Conclusion Ultrasound-guided needle biopsy is highly consistent with postoperative pathological diagnosis,which can effectively improve the diagnostic sensitivity and specificity.At the same time,the puncture has high success rate and good application safety.It has important value in early diagnosis and differential diagnosis of breast masses.
