下肢运动时机前移对妇科肿瘤术后下肢深静脉血栓形成的影响

摘　　要：目的:探讨下肢运动时机前移对妇科肿瘤术后下肢深静脉血栓形成(DVT)的影响。方法:将2018年6月1日~2020年8月31日收治的280例妇科肿瘤手术患者采用随机数表法分为观察组和对照组各140例,两组均实施围术期常规护理,观察组进行下肢运动时机前移训练,对照组采用常规下肢运动训练。比较两组干预效果。结果:观察组术后第3天患侧酸、麻、胀痛评分均低于对照组(P<0.05);护理后,观察组患者凝血功能指标活化部分凝血活酶时间(aPTT)、凝血酶原时间(PT)均高于对照组(P<0.05),纤维蛋白原(Fbg)浓度低于对照组(P<0.05);护理后,两组焦虑自评量表(SAS)、抑郁自评量表(SDS)评分均低于同组护理前(P<0.05),且观察组低于对照组(P<0.05);观察组术后淋巴囊肿、局部感染、尿瘘、DVT发生率均低于对照组(P<0.05)。结论:下肢运动时机前移可有效改善妇科肿瘤患者术后下肢血流和凝血功能,降低并发症发生率,缓解患者不良情绪。Objective:To explore the effect of advancing lower limb exercise on the deep venous thrombosis(DVT)after gynecological tumor surgery.Methods:A total of 280 patients undergoing gynecological tumor surgery admitted during June 1,2018 to August 31,2020 were divided into the observation group and the control group by random number table method,140 cases in each group.Both groups adopyed perioperatie routine care,the observation group underwent advancing lower limb exercise,while the control group underwent routine exercise training.And comparison was made between the two groups on intervention effect.Results:The soreness,numbness and pain scores of the affected side on the 3rd day after operation were lower in the observation group than those in the control group(P<0.05).After nursing,the coagulation function indexes such as activated partial thromboplastin time(aPTT)and prothrombin time(PT)in the observation group were higher than those in the control group(P<0.05),the concentration of fibrinogen(Fbg)was lower in the observation hroup than that in the the control group(P<0.05),The SAS and SDS scores were lower in both groups before intervention(P<0.05),and lower in the observation group than those in the control group(P<0.05).The incidence of postoperative lymphocele,local infection,urinary fistula and DVT in the observation group was lower than that in the control group(P<0.05).Conclusion:Advancing lower limb exercise can effectively improve lower limb blood flow and coagulation function in patients with gynecological tumors after surgery,reduce the incidence of complications,and relieve their unhealthy emotions..
