摘 要:目的探究骨科老年患者血清维生素D水平与季节变化的关系及其引起的骨代谢变化特点。方法调取2014年11月至2020年1月北京医院骨科1831例老年患者血清25羟基维生素D(25-OH-D)、人血清和血浆中总Ⅰ型前胶原氨基端延长肽(tP1NP)、N端中段骨钙素(OCN)以及Ⅰ型胶原羧基端肽β特殊序列(β-CTX)检验结果,采用Mann-Whitney U检验分析不同季节患者血清25-OH-D及骨代谢指标水平特点,以及不同性别、不同就诊类型患者之间指标的差异。结果血清25-OH-D水平无论季节变化,均处于维生素D缺乏状态,夏、秋两季血清25-OH-D水平明显高于冬、春两季(P<0.05),且夏、秋两季男性水平明显高于女性(P<0.05);门诊患者一年四季血清25-OH-D水平均高于住院患者(P<0.001),tP1NP及β-CTX水平均低于住院患者(P<0.05)。结论骨科老年患者维生素D水平夏秋季节最高;性别及就诊类型对维生素D和骨代谢指标的影响存在相关季节特点。Objective To investigate whether seasonal variation in serum 25-OH-D and bone metabolism levels among older orthopedic patients.Methods Serum 25-hydroxyvitamin D(25-OH-D),total procollagen type 1 amino-terminal propeptide(tP1NP),osteocalcin(OCN),andβ-Crosslaps(β-CTX)was collected from elderly patients in the Department of Orthopedics of Beijing Hospital from November 2014 to January 2020.Total of 1831 samples were analyzed by Mann-whitney U test to compare the levels of serum 25-OH-D,bone metabolism markers among different seasons,as well as the effect of gender and in or out patient.Results The level of serum 25-OH-D in summer and autumn was significantly higher than that in winter and spring(P<0.05),and the level of serum 25-OH-D in men was significantly higher than that in women(P<0.05).The serum 25-OH-D level in outpatients was higher than that in inpatients(P<0.001),and the levels of tP1NP andβ-CTX were lower in inpatients(P<0.05)in all seasons.Conclusions The serum vitamin D level of elderly patients in orthopedics department is higher in summer and autumn.The effects of gender and in or out patient on vitamin D and bone metabolism markers are seasonal.
