某院2020年368例住院患者万古霉素使用合理性评价

摘　　要：目的对某医院2020年万古霉素使用情况进行合理性评价,促进万古霉素临床合理使用。方法收集368例住院患者使用万古霉素的病历,对患者的给药剂量、给药途径、疗程、联合用药等情况进行统计分析。结果 368例住院患者中,以血液内科使用万古霉素的比率最高,占比为24.46%;微生物样本送检率为78.72%;万古霉素不合理使用主要表现在溶媒浓度不适宜121例(32.88%),疗程不适宜24例(6.52%),给药剂量不适宜19例(5.16%)。结论 2020年该院万古霉素使用基本合理,但还应加强微生物送检,提高对万古霉素溶媒浓度的把握,采取有效措施促进万古霉素合理使用。Objective To evaluate the rationality of vancomycin use in a hospital in 2020,so as to promote the rational clinical use of vancomycin.Methods The medical records of 368 inpatients using vancomycin were collected,and the dose,route of administration,duration of treatment,and combination of medication for patients were statistically analyzed.Results Among 368 inpatients,the proportion of vancomycin in hematology department was the highest,accounting for 24.46%.The submission rate of microbiological sample was 78.72%;the unreasonable use of vancomycin was mainly manifested in 121 cases(32.88%) with inappropriate solvent concentration,24 cases(6.52%) with inappropriate course of treatment,and 19 cases(5.16%) with inappropriate dosage.Conclusion The use of vancomycin in 2020 is basiscally reasonable,but it is necessary to strengthen the detection of microorganism and improve the control of the concentration of vancomycin solvent.Effective measures should be taken to promote the rational use of vancomycin.
