摘 要:目的检测基质金属蛋白酶-2(MMP-2)和基质金属蛋白酶抑制因子-2(TIMP-2)在退行性变主动脉三叶瓣的表达,探讨其在退行性变主动脉三叶瓣中的作用与意义。方法选取2015年6月至2018年3月天津胸科医院心外科住院患者因主动脉瓣狭窄行手术切除的严重钙化的主动脉三叶瓣标本22例为研究组,其中男性14例,平均年龄(66±6)岁,因夹层动脉瘤行Bentall手术的正常三叶式主动脉瓣标本6例为对照组,其中男性4例,平均年龄(43±5)岁;采用HE染色和免疫组织化学染色方法,观察退行性变主动脉瓣中MMP-2、TIMP-2的表达并对其进行分析。结果对照组瓣膜与研究组瓣膜的钙化部位的MMP-2表达具有显著差异(P=0.003),而对照组瓣膜与研究组瓣膜的非钙化部位,研究组瓣膜的钙化部位与非钙化部位的MMP-2表达无差异(分别为P=0.218,P=0.071);对照组瓣膜与研究组瓣膜的非钙化部位,对照组瓣膜与研究组瓣膜的钙化部位,研究组瓣膜的非钙化部位与钙化部位TIMP-2的表达无差异(分别为P=0.110,P=0.07,P=0.968),无统计学意义。结论退行性变主动脉瓣中MMP-2表达水平增高,MMP-2可能参与退行性变主动脉瓣细胞外基质(ECM)重构,导致主动脉瓣结构和功能障碍,在主动脉瓣退行性变的发生发展中起重要作用。Objective To explore the expression of matrix metalloproteinase-2(MMP-2) and matrix metalloproteinase-inhibitor 2(TIMP-2) and their roles and significance in degenerative aortic triclovalve. Methods From June 2015 to March 2018, 22 cases of severely calcified aortic triclovalve specimens were selected as the study group, including 14 males with an average age of(66±6) years. Six normal three-lobe aortic valve specimens treated with Bentall for dissecting aneurysm were used as control group, including 4 males, with an average age of(43±5) years. The expression of MMP-2 and TIMP-2 in the degenerative aortic valve was observed and analyzed by HE staining and immunohistochemical staining. Results There was a significant difference in the expression of MMP-2 in the calcified parts of the valve between the control group and the study group(P=0.003), but there was no difference in the expression of MMP-2 in the calcified parts of the valve in the study group and the noncalcified parts of the valve in the study group(P=0.218, P=0.071). There was no significant difference in the expression of TIMP-2 between the non-calcified parts of 2 groups(P=0.110, P=0.07, P=0.968). Conclusion The expression level of MMP-2 in the degenerative aortic valve is increased. MMP-2 may be involved in the reconstruction of the extracellular matrix(ECM) of the degenerative aortic valve, leading to the structural and functional dysfunction of the aortic valve, and play an important role in the occurrence and development of the degenerative aortic valve.
