邻里社会资本、城市医疗卫生服务体系与自评健康——基于中国劳动力动态调查的实证研究

摘　　要：从仅依靠卫生健康系统转向社会整体联动,实现将健康融入所有政策,是健康中国战略实施的重点。本研究基于中国劳动力动态调查数据和多层次随机截距模型,探讨邻里社会资本、城市医疗卫生服务体系对城市居民健康状况的影响效应,以及健康促进的多层次作用机理。研究发现,个人层次和社区层次的邻里社会资本都有利于提高城市居民的健康水平。邻里社会资本不同层次之间的交互作用对健康具有累积效应,这说明聚合层次社会资本对健康促进效应并不具有普遍性,而是以个体社会资本的构建为前提。在医疗卫生服务体系不完善的城市,个人层次的邻里社会资本对健康促进有补偿性的影响效应。基于此,人口健康促进政策需摆脱单纯依靠医疗卫生体系的思路,将培育邻里社会资本纳入持久性的健康战略体系。The key point of Healthy China Strategy has shifted from only relying on the health system to the overall linkage of the society, thus it is necessary to integrate health improvement into all social policies. Based on the data from CLDS and multilevel random intercept model, this paper discusses the effect and mechanism of neighbourhood social capital and urban health service system on residents’ health, and further expands the understanding of the health promotion mechanism. Firstly, it is found that neighborhood social capitals of individual level and the community level is conducive to improving the health of urban residents. Secondly, the interaction at different levels of neighborhood social capitals has a cumulative effect on health, which shows that the effect of social capital at the collective level on health promotion is not universal, but based on the construction of individual social capital. Finally, in the places with low supply efficiency of urban health service system, neighborhood social capital at the individual level can play a compensatory role in promoting the health of residents. Thus health policy intervention needs to go beyond relying only on the health system, and integrate the neighborhood social capital into the sustainable health strategy..
