雷忠义运用痰瘀相关理论治疗冠心病经验

摘　　要：总结雷忠义教授从痰瘀相关理论论治冠心病的临证经验。认为痰浊、瘀血常相兼为病,提出痰浊说与瘀血说融为一体的痰瘀互结说,并根据冠心病患者临床具体表现进行辨证施治。临证需痰瘀并治者,法以化痰宣痹、活血化瘀为主,予舒心片治疗;需痰瘀虚并治者,法以益气养阴、活血化瘀、祛痰通络为主,予养心活血汤;需痰瘀毒并治者,法以涤痰化浊、活血化瘀、清热解毒为主,予丹曲饮治疗;需痰瘀毒风并治者,法以补益气阴、祛风宣痹或化痰行瘀、熄风定悸为主,方以养心活血汤加祛风之品。This paper summarized LEI Zhongyi’s experience in treating coronary heart disease(CHD)with phlegm and stasis related theory.As it is common that concurrent phlegm turbidity and blood stasis together lead to diseases,the theory of the binding of phlegm and stasis is put forward based on phlegm turbidity theory and blood stasis theory,which can be used to guide the syndrome differentiation and treatment of CHD according to the clinical manifestations.When there is both phlegm and stasis,it is suggested to dissolve phlegm and diffuse bi(痹),invigorate blood and dissolve stasis,and Shuxin Tablet(舒心片)can be used;when there is a comorbidity of deficiency with phlegm and stasis,the methods of boosting qi and nourishing yin,invigorating blood and dissolving stasis,dispelling phlegm and unblocking the collaterals are mainly used,and Yangxin Huoxue Decoction(养心活血汤)is suggested;when there is a comorbidity of toxins with phlegm and stasis,the methods of clearing up phlegm and removing turbidity,invigorating blood and dissolving stasis,clearing heat and resolving toxins should be used,and Danqu Beverage(丹曲饮)is advised;when there is concurrent phlegm,stasis,toxins and wind,it is suggested either to supplement and boost qi and yin,dispel wind and diffuse bi,or to dissolve phlegm and move stasis,extinguish wind and relieve palpitations,and Yangxin Huoxue Decoction with wind-expelling formula should be used.
