摘 要:目的探讨健脾益气摄血方治疗原发免疫性血小板减少症(ITP)的可能效应机制。方法 30只BALB/c小鼠随机分为正常组,模型组,强的松组和健脾益气摄血中、高剂量组,每组6只。除正常组外,其余各组小鼠使用豚鼠抗小鼠血小板血清(GP-APS)注射法建立ITP小鼠模型。造模成功后第8天,正常组、模型组均予0.1 ml/10 g生理盐水灌胃,强的松组给予浓度为2 mg/ml强的松溶液0.1 ml/10 g灌胃,健脾益气摄血组中、高剂量组分别给予健脾益气摄血方颗粒剂4.1、8.2 mg/g灌胃。各组均每日1次,共灌胃8天。使用免疫浊度法检测小鼠肠黏液中分泌型免疫球蛋白A (sIgA)含量,免疫组化法检测小鼠肠系膜淋巴结淋巴细胞P53蛋白表达,TUNEL法检测小鼠肠系膜淋巴结淋巴细胞凋亡率,流式细胞术观察小鼠肠系膜淋巴结淋巴细胞周期的变化。结果与正常组比较,模型组小鼠肠黏液中sIgA含量降低,肠系膜淋巴结淋巴细胞P53蛋白表达升高,淋巴细胞凋亡率升高(P<0.01);与模型组比较,强的松组及健脾益气摄血中、高剂量组sIgA均升高,p53蛋白表达降低,淋巴细胞凋亡率均显著降低(P<0.05或P<0.01);健脾益气摄血高剂量组sIgA含量明显高于健脾益气摄血中剂量组和强的松组(P<0.01),健脾益气摄血高剂量组和强的松组淋巴细胞凋亡率均低于健脾益气摄血中剂量组(P<0.01);各组小鼠淋巴细胞周期分布差异均无统计学意义(P>0.05)。结论健脾益气摄血方治疗ITP效应机制可能与调控肠黏液中sIgA含量及肠系膜淋巴结淋巴细胞中P53蛋白表达,降低淋巴细胞凋亡率,改善肠道免疫功能有关,且具有一定的量效关系。Objective To explore the possible mechanism of Jianpi Yiqi Shexue Formula(健脾益气摄血方,JYSF)in the treatment of primary immune thrombocytopenia(ITP).Methods Thirty BALB/c mice were randomly divided into control group,model group,prednisone group,JYSF moderate-dose and high-dose groups,with six mice in each group.Except for the normal group,mice were injected with guinea pig anti-mouse platelet serum(GP-APS)to establish the ITP model.On the 8th day after successful modelling,the control group and the model group were given 0.1 ml/10 g of normal saline by gavage;the predinsone group was given 0.1 ml/10 g of prednisone solution with a 2 mg/ml concentration by gavage;and the JYSF moderate-dose and high-dose groups were given 4.1 mg/g and 8.2 mg/g JYSF granules by gavage,respectively.All the groups were administered once daily for eight days.The content of secretory immunoglobulin A(sIgA)in mouse intestinal mucus was detected by immunoturbidimetry;P53 protein expression in mouse mesenteric lymph node lymphocytes was detected by immunohistochemistry;the apoptosis rate of the mesenteric lymph node lymphocytes was detected by TUNEL;and the change of lymphocyte cycle in mesenteric lymph node was detected by flow cytometry.Results Compared to the normal group,the model group significantly decreased the content of sIgA in intestinal mucus,increased the expression of P53 protein in mesenteric lymph node lymphocytes,and increased the lymphocyte apoptosis rate(P<0.01).Compared to the model group,the other three groups significantly increased the slgA content(P<0.01),as well as decreased the expression of p53 protein and the lymphocyte apoptosis rate(P<0.05 or P<0.01).When compared among the prednisone group and the JYSF groups,it was found that the sIgA content was highest in the JYSF high-dose group(P<0.01),and the lymphocyte apoptosis rate was highest in the JYSF moderate-dose group(P<0.01).There was no significant difference among the groups in lymphocyte cycle(P>0.05).Conclusion The mechanism of JYSF in the treatm
