通络止痛散1号联合糖麻痛消汤治疗糖尿病周围神经病变的效果评价

摘　　要：目的:探究用通络止痛散1号联合糖麻痛消汤治疗糖尿病周围神经病变的效果。方法:选取枣庄市峄城区人民医院2018年4月至2020年4月收治的糖尿病周围神经病变患者90例作为研究对象。采用随机数表法将其分为比较组与联合组,每组各45例患者。对两组患者均进行常规治疗。在此基础上,为比较组患者采用糖麻痛消汤进行治疗,为联合组患者采用通络止痛散1号联合糖麻痛消汤进行治疗。然后对比两组患者治疗前后的中医证候积分(包括四肢疼痛、四肢发凉、感觉减退、肢软麻木、肌肉萎缩的积分)、血糖指标、神经传导速度和不良反应的发生情况。结果:(1)治疗后,联合组患者四肢疼痛、四肢发凉、感觉减退、肢软麻木、肌肉萎缩的积分均低于比较组患者,差异有统计学意义(P<0.05)。(2)治疗后,联合组患者空腹血糖、糖化血红蛋白的水平均低于比较组患者,差异有统计学意义(P<0.05)。(3)治疗后,联合组患者正中神经、腓总神经的传导速度均快于比较组患者,差异有统计学意义(P<0.05)。(4)治疗后,两组患者均未出现明显的不良反应。结论:用通络止痛散1号联合糖麻痛消汤治疗糖尿病周围神经病变可取得良好的临床效果,能够有效地减轻患者的临床症状及体征,改善其血糖指标,提高其神经传导速度,且安全性较高。Objective:To explore the effect of Tongluo Zhitong SAN no.1 combined with Tangma Tongxiao Decoction in the treatment of diabetic peripheral neuropathy.Methods:ninety patients with diabetic peripheral neuropathy in Zaozhuang Yicheng People’s Hospital from April 2018 to April 2020 were selected as the subjects.They were divided into comparison group and combined group by random table method,with 45 patients in each group.Routine treatment was performed in both groups.On this basis,the comparison group was treated with Tangma Tongxiao Decoction,and the combined group was treated with Tongluo Zhitong Powder no.1 combined with Tangma Tongxiao Decoction.Then,the scores of TCM syndromes(including the scores of limb pain,limb cooling,hypoesthesia,limb soft numbness and muscle atrophy),blood glucose indicators,nerve conduction velocity and the occurrence of adverse reactions were compared between the two groups before and after treatment.Results:(1)After treatment,the scores of limb pain,limb cooling,hypoesthesia,limb soft numbness and muscle atrophy in the combined group were lower than those in the comparison group,with statistical significance(P<0.05).(2)After treatment,the levels of fasting blood glucose and hba1c in combined group were lower than those in comparison group,the differences were statistically significant(P<0.05).(3)After treatment,the conduction velocity of median nerve and common peroneal nerve in combined group was faster than that in comparison group,with statistical significance(P<0.05).(4)After treatment,there were no obvious adverse reactions in 2 groups.Conclusion:Tongluo Zhitong SAN 1 combined with Tangma Tongxiao Decoction can achieve good clinical effect in the treatment of diabetic peripheral neuropathy,which can effectively reduce the clinical symptoms and signs of patients,improve their blood glucose indicators,improve their nerve conduction velocity,and has a high safety.
