摘　　要：目的探讨HEART五步沟通模式在突发性耳聋(SHL)患者中的护理效果及对心理波动的影响。方法选择2019年6月至2020年6月深圳市龙岗区耳鼻咽喉医院SHL患者200例作为对象。采用随机数字法分为对照组和观察组, 各100例。对照组采用常规护理, 观察组在对照组基础上联合HEART五步沟通模式, 2 w干预后对患者效果进行评估, 比较两组焦虑、抑郁自评量表评分、纯音听力检查结果、满意度及依从性。结果观察组护理后2 w抑郁自评量表(SDS)、焦虑自评量表(SAS)评分均低于对照组, 差异均有统计学意义(均P<0.05);观察组护理后1 d、7 d、10 d、14 d纯音听力低于对照组, 差异有统计学意义(P<0.05);观察组HEART五步沟通模式干预后2 w护理方法、护理效果、护理内容满意度、遵医治疗、定期复查及康复锻炼依从性高于对照组, 差异有统计学意义(均P<0.05)。结论 HEART五步沟通模式用于SHL患者中能降低患者心理波动, 提高患者纯音听力, 可提高患者满意度、依从性, 值得推广应用。Objective To explore the nursing effect of HEART five-step communication mode in patients with sensorineural hearing loss and its effect on psychological fluctuations.Methods A total of 200 SHL patients in this hospital from June 2019 to June 2020 were selected as subjects.Patients were divided into control group(100 cases)and observation group(100 cases)by random number method.The control group received routine nursing,and the observation group was combined with HEART five-step communication mode on the basis of control group.The effect of 2 weeks intervention was evaluated.The scores of Self-rating Depression Scale(SDS)and Self-rating Anxiety Scale(SAS),pure tone hearing test results,satisfaction and compliance were compared between the two groups.Results The SDS and SAS scores in the observation group at 2 weeks after nursing were lower than those in the control group,and the differences were statistically significant(P<0.05).The pure tone hearing of observation group was lower than that of control group at 1 day,7 days,10 days and 14 days after nursing,and the difference was statistically significant(P<0.05).The observation group was higher than the control group in nursing method,nursing effect,nursing content satisfaction,compliance with medical treatment,regular review and rehabilitation exercise 2 weeks after intervention with HEART five-step communication mode,the difference was statistically significant(P<0.05).Conclusions The HEART five-step communication mode can reduce the psychological fluctuations of patients with SHL,improve the pure tone hearing of patients,and lift patient satisfaction and compliance.It is worth popularizing and applying.
