正交设计筛选防栓方精减方预防血栓形成的主要药味

摘　　要：目的观察防栓方各拆方组预防妇科盆腔手术后患者下肢深静脉血栓(leg deepvenousthrombosis,LDVT)形成的效果,筛选出其预防血栓形成的主要药味。方法通过正交试验设计将防栓方拆方分为12个组,选取西南医科大学附属中医医院2019年3月至2020年4月具有发生下肢深静脉血栓中高危因素的妇科盆腔手术患者为纳入对象,每组按组别口服不同组方的中药汤剂,一日三次,每次一格,术后24小时后开始服用,在术后检测相关指标,观察防栓方各拆方组预防LDVT发生的疗效,筛选出防栓方的精减方。结果防栓方中具有预防血栓形成的主要药味是丹参、赤芍、当归、桃仁、红花、川芎、川牛膝,而黄芪、党参、枳实、酒大黄是负作用因素。结论防栓方精减方药物组成:丹参、赤芍、当归、桃仁、红花、川芎、川牛膝。Objective To observe the effect of each component of antithrombotic prescription in preventing leg deep vein throm⁃bosis(LDVT)after gynecologic pelvic surgery,and to screen for main components with antithrombotic effects.Methods According to the orthogonal design,the components of antithrombotic prescription were divided into 12 groups;patients who had a gynecologic pelvic surgery in the Affiliated Traditional Chinese Medicine Hospital of Southwest Medical University from March 2019 to April 2020 and had high risk factors for LDVT were enrolled as subjects;traditional Chinese medicine decoctions of different components were orally administered(1 compartment,3 times a day)in each group 24 hours after surgery;related indicators were measured after surgery to ob⁃serve the effect of each component of antithrombotic prescription in preventing LDVT and to screen for the main components of anti⁃thrombotic prescription.Results The main components with antithrombotic effects in the antithrombotic prescription were Salvia milt⁃iorrhiza,Radix Paeoniae Rubra,Angelica sinensis,peach seed,Carthamus tinctorius,Rhizoma Chuanxiong,and Radix Achyranthis Bidentatae;Astragalus membranaceus,Codonopsis pilosula,Fructus Aurantii Immaturus,and Rheum officinale were negative factors.Conclusion The reduced antithrombotic prescription is composed of Salvia miltiorrhiza,Radix Paeoniae Rubra,Angelica sinensis,peach seed,Carthamus tinctorius,Rhizoma Chuanxiong,and Radix Achyranthis Bidentatae.
