摘  要:目的:探讨如何减少多发性骨髓瘤患者应用达雷妥尤单抗对输血相容性检测干扰,保障患者输血安全。方法:将患者、供血者不规则抗体筛查的红细胞,用二硫苏糖醇进行处理,并将处理前后的红细胞用经典和微柱卡间接抗人球蛋白法(IAT)、凝聚胺法,进行输血相容性检测。结果:用二硫苏糖醇处理红细胞前后,经典IAT、微柱卡IAT的结果差异明显,由阳性变为阴性,经典IAT检测过程中出现溶血,凝聚胺法的结果一致,抗-E抗体均未漏检。结论:用二硫苏糖醇处理红细胞,可有效的消除达雷妥尤单抗对输血相容性检测的干扰,微柱卡间接抗人球蛋白法、凝聚胺法更适用于输血科对应用达雷妥尤单抗治疗的患者进行输血相容性检测。Objective: To explore how to reduce the interference of Daratumumab(Dara) on blood transfusion compatibility test for patients with multiple myeloma and ensure the safety of blood transfusion. Methods: Dithiothreitol(DTT) was used to treat erythrocytes from patients and blood donors and antibody screening red blood cell kit, the transfusion compatibility test of pre-treated and post-treated red blood cells were tested by classical method and microcolumn indirect antiglobulin method and manual polybrene method. Results: Before and after treatment with DTT, the results of classical indirect antiglobulin method and microcolumn indirect antiglobulin method were obviously different, the result changed from positive to negative, hemolysis occurred during the classical indirect antiglobulin test, the results of manual polybrene method were consistent, no anti-E antibodies were missed. Conclusion: Treatment with DTT can effectively eliminate the interference of Dara on blood transfusion compatibility test. Microcolumn indirect antiglobulin method and manual polybrene method may be more suitable for blood transfusion compatibility testing in patients treated with Dara in the department of blood transfusion.
