摘 要:目的:鉴定Dib阴性孕妇不规则抗体特异性并筛选配合血液,探讨稀有血型孕妇围产期供血保障策略。方法:对医院不规则抗体筛查阳性的孕妇进行免疫血清学抗体鉴定,对照筛选细胞及谱细胞格局,鉴定为抗Dib稀有血型抗体后,在亲属血源及稀有血型库寻找配血相合的血液或尽早考虑自体输血。结果:2例围产期孕妇不规则抗体鉴定为抗Dib抗体,亲属血源中均存在鉴定为同血型Dib阴性的血液,且与孕妇主侧配血相合;另有稀有血型库存2 U Dib阴性冻存血液;筛选的血液和冻存血液后续被用于孕妇输血及其新生儿溶血病的治疗。结论:重视围产期孕妇不规则抗体检测及供血保障,持续更新并加强稀有血型库建设,可以有效改善孕妇及新生儿紧急输血需求,预防或治疗母胎血型同种免疫诱发的胎儿/新生儿溶血病。Objective: To explore the identification of irregular antibodies and the strategy of blood transfusion in pregnant women with Diego rare blood group in perinatal period, so as to provide the oretical and practical support for the study of maternal-fetal blood group alloimmunity in pregnant women with rare blood group. Methods: The pregnant women with positive irregular antibody screening in hospital were identified by immuno-serological antibody identification, compared with the cell pattern and spectral cell pattern, identified the immunological characteristics of the antibody, identified as anti-Dib rare blood group antibody, looked for matched blood from relatives and rare blood group bank, or considered autotransfusion as soon as possible. Results: The irregular antibodies of 2 perinatal pregnant women were identified as anti-Dib antibodies, and all the blood sources of their relatives were identified as Dib-negative blood of the same blood type, which coincided with the main blood matching of the pregnant women, and 2 U Dib-negative frozen blood was stored in the rare blood type bank. The screened blood and frozen blood were subsequently used for the treatment of maternal blood transfusion and neonatal hemolytic disease. Conclusion: Strengthening the detection of irregular antibodies in perinatal pregnant women, paying attention to the guarantee of blood supply of pregnant women with rare blood type, and constantly updating and strengthening the construction of rare blood group bank can effectively improve the emergency blood transfusion needs of pregnant women and newborns. It can effectively prevent or treat fetal/neonatal hemolytic disease induced by maternal-fetal blood group alloimmunity.
