摘 要:目的探讨依达拉奉联合脑苷肌肽对重症颅脑损伤患者神经功能及血清炎症因子水平的影响。方法选取2018年8月—2020年10月平顶山市某医院收治的88例重症颅脑损伤患者为研究对象,采用随机数字表法分为对照组和观察组,每组44例。对照组患者在对症治疗基础上给予脑苷肌肽治疗,观察组患者在对照组基础上联用依达拉奉治疗,2组均连续治疗14 d。比较2组患者的临床疗效,治疗前后神经功能及血清白介素-6(IL-6)、白介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平。结果观察组患者的治疗总有效率为97.73%,高于对照组患者的81.82%,差异有统计学意义(χ^(2)=4.456,P=0.035)。2组患者治疗后美国国立卫生研究院卒中量表(NIHSS)评分低于治疗前且观察组低于对照组,日常生活能力量表(ADL)及上肢运动功能量表(Fugl-Meyer)评分高于治疗前且观察组高于对照组,差异均有统计学意义(P<0.05)。2组患者治疗后血清IL-6、IL-8、TNF-α水平均低于治疗前,且观察组低于对照组,差异均有统计学意义(P<0.05)。结论依达拉奉联合脑苷肌肽可有效缓解重症颅脑损伤患者临床症状,降低患者血清炎症因子水平,改善患者神经功能,有助于病情恢复。Objective To investigate the effects of edaravone combined with cattle encephalon glycoside and ignotin on neurological function and serum inflammatory factor levels in patients with severe craniocerebral injury.Methods Eighty-eight patients with severe craniocerebral injury admitted to a hospital in Pingdingshan from August 2018 to October 2020 were selected and divided into a control group and an observation group using the random number table method,with 44 cases in each group.The patients in the control group were treated with cattle encephalon glycoside and ignotin on the basis of symptomatic treatment,and the patients in the observation group were treated with edaravone on the basis of the control group.Both groups were treated continuously for 14 d.The clinical efficacy,neurological function and serum interleukin-6(IL-6),interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α)levels of the patients in the two groups were compared before and after treatment.Results The total effective rate of treatment in the observation group was 97.73%,which was higher than that in the control group(81.82%),and the difference was statistically significant(χ^(2)=4.456,P=0.035).the NIHSS score in the two groups was lower than that before treatment and lower in the observation group than in the control group,and the ADL and Fugl-Meyer scores were higher than that before treatment and higher in the observation group than in the control group,all with statistically significant differences(P<0.05).The serum levels of IL-6,IL-8 and TNF-αwere lower in both groups than before treatment and lower in the observation group than in the control group,with statistically significant differences(P<0.05).Conclusion Edaravone combined with cattle encephalon glycoside and ignotin can effectively relieve the clinical symptoms of patients with severe craniocerebral injury,reduce the levels of serum inflammatory factors,improve the neurological function of patients,and contribute to the recovery of the disease.
