摘 要:目的探讨分级运动训练对缺血性脑卒中患者神经功能、生活自理能力的影响。方法选取2019年10月—2020年7月广东省珠海市某三甲医院神经内科收治的116例急性期缺血性脑卒中住院患者为研究对象。将入组的116例患者用SPSS 22.0软件产生的随机数字分为观察组和对照组,每组58例。对照组患者采取常规康复训练,观察组患者给予分级运动训练。干预前后采用美国国立卫生研究院卒中量表(NIHSS)评估患者神经功能缺损程度、Barthel指数评定量表评估患者日常生活能力、Fugl-Meyer量表(FMA)评估患者的运动功能。结果干预前,2组患者NIHSS评分比较,差异无统计学意义(P>0.05)。干预后,2组患者NIHSS评分均低于干预前,且观察组低于对照组,差异均有统计学意义(P<0.05)。干预前,2组患者Barthel、FMA评分比较,差异无统计学意义(P>0.05)。干预后,2组患者Barthel、FMA评分均高于干预前,且观察组高于对照组,差异均有统计学意义(P<0.05)。结论对缺血性脑卒中患者实施分级运动训练,有助于恢复患者神经功能,提高患者的运动和生活能力,值得临床推广应用。Objective To explore the effect of graded exercise on the neurological function and self-care ability of patients with ischemic stroke.Methods One hundred and sixteen inpatients with acute-phase ischemic stroke admitted to the Department of Neurology of a tertiary hospital in Zhuhai,Guangdong Province,from October 2019 to July 2020 were selected for the study.The 116 patients were divided into observation and control groups using random numbers generated by SPSS 22.0 software,with 58 cases in each group.Patients in the control group were given conventional rehabilitation training,while patients in the observation group were given graded exercise training.The National Institutes of Health Stroke Scale(NIHSS)was used to assess the degree of neurological deficits,the Barthel Index scale to assess the patients′ability of daily living,and the Fugl-Meyer assessment(FMA)to assess the patients′motor function before and after the intervention.Results Before the intervention,there was no statistically significant difference between the NIHSS scores in the 2 groups(P>0.05).After the intervention,the NIHSS scores of patients in the 2 groups were lower than those before the intervention and lower in the observation group than in the control group,with statistically significant differences(P<0.05).Before the intervention,there was no statistically significant difference between the Barthel and FMA scores in the 2 groups(P>0.05).After the intervention,Barthel and FMA scores were higher in the 2 groups than before the intervention,and the observation group was higher than the control group,with statistically significant differences(P<0.05).Conclusion The implementation of graded exercise rehabilitation training for ischemic stroke patients can help restore neurological function and improve patients′motor function and living ability,and it is worthy of clinical application.
