口炎清颗粒治疗慢性牙周炎临床疗效的系统评价和Meta分析

摘　　要：目的系统评价口炎清颗粒治疗慢性牙周炎的临床疗效和安全性。方法算机检索CNKI、CBM、VIP、Wan Fang Data、Cochrane图书馆、PubMed等数据库,纳入相关随机对照试验后进行评价,并采用Revman 5.3软件分析提取数据。结果最终纳入6篇RCTs,共计713例受试者。加用口炎清组治疗慢性牙周炎的临床有效率高于对照组(RR=1.15,95%CI=[1.08,1.22],P<0.05);治疗后,可明显降低牙周炎患者的牙周探诊深度(WMD=-0.66,95%CI=[-0.86,-0.46],Z=6.60,P<0.05),可明显降低牙周炎患者的菌斑指数(WMD=-0.58,95%CI=[-0.98,-0.18],Z=2.85,P<0.05),可明显降低牙周炎患者的龈沟出血指数(WMD=-0.50,95%CI=[-0.66,-0.34],Z=6.18,P<0.05),可明显降低牙周炎患者的附着丧失水平(WMD=-0.67,95%CI=[-0.85,-0.50],Z=7.61,P<0.05);各组不良反应发生率均无统计学意义(RR=4.50,95%CI=[0.99,20.52],P=0.90)。结论加用口炎清颗粒改善慢性牙周炎的临床疗效优于单纯使用西药的常规治疗,在牙周基础治疗的前提下加用口炎清颗粒可有效缓解慢性牙周炎患者的症状,中西医结合治疗可促进牙周炎的转归,对PD、PLI、SBI、AL的改善有效,不良反应发生率未见与其他药物有明显差异,同时建议补充更多高水平的相关研究为临床提供更多循证医学证据。Objective To systematically evaluate the efficacy and safety of Kouyanqing granules in the treatment of chronic periodontitis.Methods Computer searched databases of CNKI,CBM,VIP,Wan Fang Data,Cochrane Library,PubMed,etc,were included in relevant randomized controlled trials for evaluation,and Revman 5.3 software was used to analyze and extract data.Results Finally,6 RCTs were included,with a total of 713 subjects.The clinical effective rate of the treatment of chronic periodontitis in the Kouyanqing group was higher than that of the control group(RR=1.15,95%CI=[1.08,1.22],P<0.05).After treatment,it can significantly reduce the depth of periodontal exploration in patients with periodontitis(WMD=-0.66,95%CI=[-0.86,-0.46],Z=6.60,P<0.05),can significantly reduce the plaque index in patients with periodontitis(WMD=-0.58,95%CI=[-0.98,-0.18],Z=2.85,P<0.05),can significantly reduce the gingival crevicular bleeding index in patients with periodontitis(WMD=-0.50,95%CI=[-0.66,-0.34],Z=6.18,P<0.05),and can significantly reduce the level of attachment loss in patients with periodontitis(WMD=-0.67,95%CI=[-0.85,-0.50],Z=7.61,P<0.05).The incidence of adverse reactions in each group was not statistically significant(RR=4.50,95%CI=[0.99,20.52],P=0.90).Conclusion Adding Kouyanqing Granules to improve the clinical efficacy of chronic periodontitis is better than conventional treatment with Western medicine alone.Adding Kouyanqing Granules to basic periodontal treatment can effectively relieve the symptoms of patients with chronic periodontitis.Traditional Chinese and Western medicine combined treatment can promote the outcome of periodontitis,and is effective in improving PD,PLI,SBI,and AL.The incidence of adverse reactions is not significantly different from other drugs.At the same time,it is recommended to add more high-level related research to provide clinical better More evidence-based medical evidence.
