摘  要:目的探讨经环甲间隙裸化甲状腺上极血管入路在甲状腺切除中的临床效果。方法选取2016年1月~2018年10月之间本组医师采用经环甲间隙裸化甲状腺上极血管入路行甲状腺切除术的手术病例,分析其手术中操作情况、术后情况,总结该方法的临床效果。结果一共240例甲状腺手术,其中全甲状腺切除54例,甲状腺腺叶及峡部切除186例,所有患者均顺利完成手术,术中出血均不多,术后无一例出现发音费力、声嘶、呼吸困难、术后出血、低钙等并发症,患者对手术效果均满意。结论经环甲间隙裸化甲状腺上极血管入路这种方法具有学习简便,操作简单,术中出血少,术野清晰,能有效保护喉上神经及上甲状旁腺,是临床值得推广的一种手术操作技术。Objective To explore the clinical effect of thyroidectomy through the Cricothyroid Space naked thyroid upper pole vascular access.Methods The operative cases performed thyroidectomy by Cricothyroid Space naked thyroid upper pole vascula approach from January 2016 to October 2018 were selected to analyze the operation and post-operation conditions,and summarize the clinical effect.Results A total of 240 cases of thyroidectomy,including 54 cases of total thyroidectomy and 186 cases of thyroid gland lobe and isthmus resection,all patients successfully completed the operation,intraoperative bleeding was not much,no postoperative complications such as dysphonia,hoarseness,dyspnea,postoperative bleeding,low calcium,etc.The patients were satisfied with the results of surgery.Conclusion This method through the Cricothyroid Space naked thyroid upper pole vascular access has the advantages of simple learning,simple operation,less intraoperative blood and clear operation field.It can effectively protect the superior laryngeal nerve and parathyroid gland.It is worth popularizing in clinical practice.
