伏气消癜汤治疗血热妄行证儿童过敏性紫癜的临床研究

摘　　要：目的观察伏气消癜汤联合基础治疗对血热妄行证儿童过敏性紫癜(HSP)的临床疗效及其对免疫学指标的影响,探讨此方的作用机制与靶点。方法收集2019年10月至2021年4月黑龙江中医药大学附属第一医院儿科收治的皮肤型过敏性紫癜血热妄行证患儿60例,按照数字表法随机分为观察组和对照组,每组30例。两组患儿均给予一般常规治疗,观察组在此基础上加服伏气消癜汤,两组均治疗14 d。比较两组治疗有效率、疾病愈显率、紫癜消失时间、免疫学指标水平。结果观察组患儿总有效率略高于对照组(90%vs.86.7%),但差异无统计学意义(P>0.05);观察组患儿紫癜消失时间略短于对照组[(12.5±3.66)d vs.(13.0±3.53)d],但差异无统计学意义(P>0.05);治疗后,观察组患儿IgA水平高于对照组(P<0.05),CD4^(+)T细胞水平高于对照组(P<0.05),CD8^(+)T细胞水平低于对照组(P<0.05),而两组补体C3水平比较差异无统计学意义(P=0.14)。结论伏气消癜汤治疗血热妄行证儿童过敏性紫癜,能有效改善患儿临床症状和免疫学指标,提高临床疗效。Objective To observe the clinical efficacy of Fuqi Xiaodian Decoction combined with basic treatment in the treatment of Henoch-Schonlein purpura(HSP)in children with bleeding due to blood-heat syndrome, analyze its influence on immunological indexes, and explore the mechanism and targets.Methods Totally 60 pediatric HSP patients with bleeding due to blood-heat syndrome treated in Department of Pediatrics of the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine from October 2019 to April 2021 were divided into two groups according to random number table: observation group(n=30)and control group(n=30).All the patients received conventional treatment, and the patients in observation group additionally used Fuqi Xiaodian Decoction.The treatment lasted for 14 d.The effectiveness, recovery and effective rates, time of purpura disappearance, and levels of immune markers were compared between the two groups.Results The total effective rate of observation group was slightly higher than that of control group(90% vs.86.7%,P>0.05);the time of purpura disappearance in observation group was slightly shorter than that in control group [(12.5±3.66)d vs.(13.0±3.53)d, P>0.05];after treatment, the levels of immunoglobulin IgA and CD4^(+)in observation group were higher than those in control group(P<0.05),and the level of CD8^(+)was lower than that in control group(P<0.05);there was no significant difference in complement C3 between the two groups(P=0.14).Conclusion Fuqi Xiaodian Decoction can effectively improve the clinical symptoms and immunological indexes of the pediatric HSP patients with bleeding due to blood-heat syndrome, and enhance the clinical efficacy.
