多巴胺联合硝普钠持续静脉泵入治疗老年难治性心力衰竭的效果

摘　　要：目的观察多巴胺联合硝普钠持续静脉泵入治疗老年难治性心力衰竭的临床效果。方法选取2019年1月-2020年5月在湖北省当阳市人民医院治疗的老年难治性心力衰竭患者60例,采用随机数字表法分为观察组和对照组各30例。对照组采用常规对症治疗,观察组采用多巴胺联合硝普钠持续静脉泵入治疗,比较2组患者治疗效果、治疗前后心功能指标及不良反应。结果观察组患者的治疗总有效率高于对照组,差异有统计学意义(90.00%vs.66.67%,χ^(2)=4.812,P=0.028);治疗10 d后,2组患者的各项心功能指标均优于治疗前,且观察组优于对照组,差异均有统计学意义(P<0.01);2组患者不良反应总发生率比较差异无统计学意义(6.67%vs.13.33%,χ^(2)=0.741,P=0.389)。结论多巴胺联合硝普钠持续静脉泵入治疗老年难治性心力衰竭的疗效较好,有利于改善患者的临床症状和心功能,且具备较高的治疗安全性,值得进一步推广应用。Objective To observe the clinical effect of continuous intravenous pumping of dopamine combined with sodium nitroprusside in the treatment of senile refractory heart failure.Methods Sixty elderly patients with refractory heart failure treated in Dangyang People′s Hospital,Hubei Province from January 2019 to May 2020 were selected.The patients were divided into observation group and control group by random number table method.The control group received conventional symptomatic treatment,and the observation group received continuous intravenous pumping of dopamine combined with sodium nitroprusside.The therapeutic effect,cardiac function indexes and adverse reactions before and after treatment were compared between 2 groups.Results The total effective rate in observation group was higher than that in control group,the difference was statistically significant(90.00%vs.66.67%,χ^(2)=4.812,P=0.028).After 10 days of treatment,cardiac function indexes of 2 groups were better than before,and the observation group was better than the control group,the differences were statistically significant(P<0.01).There was no significant difference in the total incidence of adverse reactions between 2 groups(6.67%vs.13.33%,χ^(2)=0.741,P=0.389).Conclusion Continuous intravenous pumping of dopamine combined with sodium nitroprusside in the treatment of elderly patients with refractory heart failure has a good effect,is beneficial to improve the clinical symptoms and cardiac function of patients,and has a high therapeutic safety,worthy of further promotion and application.
