克霉唑阴道片联合阴道用乳杆菌活菌胶囊治疗妊娠期外阴阴道假丝酵母菌病的临床观察

摘　　要：目的观察克霉唑阴道片联合阴道用乳杆菌活菌胶囊治疗妊娠期外阴阴道假丝酵母菌病(VVC)的临床效果。方法选取2018年10月-2019年7月于云南省第三人民医院妇科门诊就诊的妊娠期VVC患者110例,采用随机数字表法分为观察组和对照组各55例。对照组给予克霉唑阴道片1片(0.5 g)置于阴道深部,第1天睡前使用;观察组在对照组治疗的基础上于第2天开始给予阴道用乳杆菌活菌胶囊1粒置于阴道深部,每晚睡前使用,连续7 d。比较2组治疗效果、临床症状缓解时间及不良反应;随访6周,比较2组复发率。结果观察组总有效率为96.36%高于对照组的85.46%(χ^(2)=6.857,P=0.032);观察组临床症状缓解时间为(2.06±0.84) d,短于对照组的(2.31±0.86) d,但差异无统计学意义(t=1.542,P=0.126);2组均无特殊用药不良反应;随访6周,观察组复发率为5.45%(3/55)低于对照组的18.18%(10/55)(χ^(2)=4.274,P=0.039)。结论克霉唑阴道片联合阴道用乳杆菌活菌胶囊治疗妊娠期VVC的效果好,可降低复发率,且无特殊不良反应,临床可推广使用。Objective To observe the clinical effect of clotrimazole vaginal tablet combined with lactobacillus vaginalis capsule in the treatment of vulvovaginal candidiasis(VVC)during pregnancy.Methods A total of 110 pregnant VVC patients who visited the department of Gynecology of the Third People′s Hospital of Yunnan Province from October 2018 to July 2019 were selected.Randomly divided into observation group and control group,55 cases in each group.Control group was given 1 clotrimazole vaginal tablet(0.5 g),placed deep in the vagina,and used before going to bed on the first day;On the basis of the treatment of the control group,the observation group was given one live lactobacillus capsule for vaginal use from the second day,placed in the deep vagina,used every night before going to bed,for consecutive 7 days.The treatment effect,clinical symptom relief time and adverse reactions of the two groups were compared.The recurrence rate of the two groups was compared after 6 weeks follow-up.Results The total effective rate in observation group was 96.36%,higher than 85.46%in control group(χ^(2)=6.857,P=0.032).The remission time of clinical symptoms in the observation group was(2.06±0.84)d,shorter than that in the control group(2.31±0.86)d,but the difference was not statistically significant(t=1.542,P=0.126).There were no special adverse drug reactions in two groups.Followed up for 6 weeks,the recurrence rate in the observation group was 5.45%(3/55),lower than that in the control group′s 18.18%(10/55)(χ^(2)=4.274,P=0.039).Conclusion Clotrimazole vaginal tablets combined with live lactobacillus capsules for vaginal use in the treatment of pregnancy VVC has a good effect,can reduce the recurrence rate,and no special adverse reactions,can be popularized in clinical use.
