中药汤剂干预2型糖尿病合并血脂异常疗效的Meta分析

摘　　要：目的采用Meta分析评价近五年中药汤剂干预2型糖尿病合并血脂异常的临床疗效并对其进行用药规律分析。方法计算机检索2016年1月—2020年12月公开发表的中药汤剂干预2型糖尿病合并血脂异常中英文随机对照研究,导入RevMan 5.3软件进行Meta分析。结果共纳入18项研究包含2036例研究对象。Meta分析结果显示联用或单用中药汤剂干预2型糖尿病合并血脂异常,可显著降低总胆固醇[MD=-0.88,95%CI(-1.19,-0.58),P<0.000 01],降低三酰甘油[MD=-0.45,95%CI(-0.48,-0.42),P<0.000 01],降低低密度脂蛋白胆固醇[MD=-0.38,95%CI(-0.68,-0.08),P=0.01],降低空腹血糖[MD=-1.02,95%CI(-1.41,-0.63),P<0.000 01],降低餐后2小时血糖[MD=-1.50,95%CI(-1.99,-1.02),P<0.000 01],降低糖化血红蛋白[MD=-1.15,95%CI(-1.46,-0.83),P<0.000 01],提高高密度脂蛋白胆固醇[MD=0.11,95%CI(0.01,0.20),P=0.03]及临床综合疗效[MD=1.23,95%CI(1.16,1.30),P<0.000 01]。所用最多的为补益药,丹参为使用频次最高的单味药。结论以补益药为主的中药汤剂干预2型糖尿病合并血脂异常疗效显著,安全性尚不明确,仍需更多高质量临床研究支持。Objective Use Meta-analysis to evaluate the clinical efficacy of traditional Chinese medicine decoction in the intervention of type 2 diabetes with dyslipidemia in the last five years and analyze its medication rules.Methods Databases were searched electronically from January 2016 to December 2020 to collect randomized controlled trials of traditional Chinese medicine decoction in the intervention for type 2 diabetes with dyslipidemia in Chinese or English and imported RevMan 5.3 software for meta-analysis.Results A total of 18 studies included 2036 patients were incorporated into study.Meta-analysis showed that the combined or single use of traditional Chinese medicine decoction can significantly reduce total cholesterol[MD=-0.88,95%CI(-1.19,-0.58),P<0.000 01],triglycerides[MD=-0.45,95%CI(-0.48,-0.42),P<0.000 01],low density lipoprotein cholesterol[MD=-0.38,95%CI(-0.68,-0.08),P=0.01],fasting blood-glucose[MD=-1.02,95%CI(-1.41,-0.63),P<0.00001],2-hour post-meal blood glucose[MD=-1.50,95%CI(-1.99,-1.02),P<0.000 01],glycosylated hemoglobin[MD=-1.15,95%CI(-1.46,-0.83),P<0.000 01],improve high density lipoprotein cholesterol[MD=0.11,95%CI(0.01,0.20),P=0.03]and clinical comprehensive efficiency[MD=1.23,95%CI(1.16,1.30),P<0.000 01].Restoratives were most frequently used,Danshen as a single herb was used mostly.Conclusion Traditional Chinese medicine decoction which based on restoratives in the intervention for type 2 diabetes with dyslipidemia is effective.The safety is not clear,and more high-quality clinical studies are expected to be included to provide evidence support.
