基于“四季脾旺不受邪”理论谈甲功正常期桥本甲状腺炎的防治

摘　　要："四季脾旺不受邪"理论出自《金匮要略》,揭露了脾与疾病产生之间的关系。甲功正常期桥本甲状腺炎(hashimoto’s thyroiditis,HT)是一种以甲状腺自身抗体呈阳性、甲状腺功能正常为特征的自身免疫性疾病。在治疗方面,西医存在一定的局限性,相比之下中医药的治疗更占据优势,在改善临床症状、降低抗体水平、预防疾病等方面都发挥出独有特色。文章基于"四季脾旺不受邪"理论,以脾为切入点论治甲功正常期桥本甲状腺炎及相关疾病,并以"固护脾胃"为中心多途径预防本病,为甲功正常期桥本甲状腺炎的中医临床防治提供了新的指导思想。The theory that "spleen flourishing in four seasons is not affected by evils" is derived from the Synopsis of The Golden Chamber,which reveals the relationship between the spleen and the occurrence of diseases.Hashimoto’s thyroiditis(HT) is an autoimmune disease characterized by positive thyroid autoantibodies and normal thyroid function.In terms of treatment,western medicine has some limitations,while traditional Chinese medicine has more advantages in terms of improving clinical symptoms,reducing antibody levels and preventing diseases.Based on the theory that "spleen flourishing in four seasons is not affected by evils",this paper took the spleen as the entry point to treat HT and related diseases in the normal stage of thyroid function,and took "strengthening and protecting the spleen and stomach" as the center to prevent this disease in multiple ways,providing a new guiding idea for the clinical prevention and treatment of HT in the normal stage of thyroid function by traditional Chinese medicine.
