实时动态超声对小儿髌下脂肪垫疝的诊断价值

摘　　要：目的总结小儿髌下脂肪垫疝的超声征象,研究实时动态超声检查对本病的诊断价值。方法收集我院55例小儿髌下脂肪垫疝的病例,回顾性分析其临床特点,并分析其静态及动态的超声图像。结果55例患儿,共计60个病变,所有病变(100%)实时动态超声均显示髌骨前外下侧皮下结节状异常回声,与髌下脂肪垫呈细条状相连,缓慢伸膝实时动态观查可见皮下异常回声逐渐变小、向关节囊内回纳,伸膝位时完全消失;46.7%的病变超声显示髌外侧支持带不连续,23.3%的病变超声显示髌外侧支持带连续,另有30.0%的病变超声不能明确髌外侧支持带连续与否。结论连续变换体位实时动态超声检查对小儿髌下脂肪垫疝具有较高诊断价值。Objective To investigate the diagnostic value of ultrasonography(US)in pediatric lateral Hoffa’s fat pad herniation.Methods A retrospective analysis was performed on the clinical and US features of 55 children with lateral Hoffa’s fat pad herniation.Results On dynamic US,all 60 lesions in 55 children presented as masses in the anterolateral aspect of the knee,which only appeared during flexion and completely resolved during extension.The lateral patellar retinacula showed focal defects(46.7%),were intact(23.3%)or indeterminate(30.0%)on US.Conclusion Postural changes on dynamic US is effective for the diagnosis of lateral Hoffa’s fat pad herniation in children.
