清热活血解毒方联合改良挂线术治疗瘘管期浆细胞性乳腺炎临床研究

摘　　要：目的:探究清热活血解毒方联合改良挂线术治疗瘘管期浆细胞性乳腺炎临床疗效。方法:选取86例瘘管期浆细胞性乳腺炎患者为研究对象,随机分为对照组(n=43)和治疗组(n=43)。对照组给予改良挂线术治疗,治疗组给予清热活血解毒方联合改良挂线术治疗,观察两组治疗前后临床症状、血清免疫球蛋白IgG、IgM、IgA以及炎症因子水平变化,评估两组临床疗效和安全性。结果:治疗后两组乳房胀痛、乳房肿胀、乳头溢液症状评分均降低,且治疗组各评分低于对照组,差异有统计学意义(均P<0.05)。治疗后两组IgG、IgM、IgA水平均降低,且治疗组低于对照组,差异有统计学意义(均P<0.05)。治疗后两组IL-1、IL-6、TNF-α水平均降低,且治疗组低于对照组,差异有统计学意义(均P<0.05)。治疗组总有效率高于对照组,而不良反应发生率、复发率低于对照组,差异有统计学意义(均P<0.05)。结论:清热活血解毒方联合改良挂线术治疗瘘管期浆细胞性乳腺炎患者能有效缓解患者临床症状,改善免疫功能,减轻炎症反应,减少不良反应,疗效较好。Objective:To explore the effect of Qingre Huoxue Jiedu prescription combined with modified suture hanging on fistula plasma cell mastitis.Methods:86 cases of fistulas with plasma cell mastitis were selected as the research objects,randomly divided into control group(n=43)and treatment group(n=43).The control group was treated with modified thread hanger,and the treatment group treated with modified thread hanger combined with Qingre Huoxue Jiedu prescription.The clinical symptoms,serum immunoglobulin IgG,IgM,IgA and inflammatory factors were observed before and after treatment.The clinical efficacy and safety performance of the two groups were evaluated.Results:After treatment,the symptoms scores of breast tenderness,breast swelling and nipple discharge in the two groups were decreased,and the scores in the treatment group were significantly lower than those in the control group,difference statistically significant(all P<0.05).After treatment,the levels of IgG,IgM and IgA in the two groups were decreased,and the levels in the treatment group were significantly lower than those in the control group,difference statistically significant(all P<0.05).After treatment,the levels of IL-1,IL-6 and TNF-αin the two groups were decreased,and the levels in the treatment group were significantly lower than those in the control group,difference statistically significant(all P<0.05).The total effective rate in the treatment group was significantly higher than that in the control group,and the incidence of adverse reactions and recurrence rate were significantly lower than those in the control group,difference statistically significant(all P<0.05).Conclusion:Qingre Huoxue Jiedu prescription combined with modified thread hanging-up can effectively relieve clinical symptoms,improve immune function of patients,reduce inflammatory response,reduce adverse reactions,with better efficacy.
