摘　　要：目的了解慢性心力衰竭患者自我报告结局与自我管理的现状,探讨两者之间的关系。方法采用便利抽样法,选取2020年1-8月于郑州2所医院就诊的314例慢性心力衰竭患者作为研究对象,采用一般资料调查表、慢性心力衰竭患者自我报告结局量表、心力衰竭自我管理量表对心力衰竭患者进行评价。结果慢性心力衰竭患者自我报告结局转化后总分为(72.78±13.09)分,处于中等水平。慢性心力衰竭患者自我管理标准化得分率为75.36%,处于中等水平。Pearson相关分析显示,慢性心力衰竭患者自我报告临床结局与自我管理的药物维度(r=0.28),饮食维度(r=0.11),心理及社会适应维度(r=0.30),症状管理维度(r=0.24)均呈正相关(P<0.01)。结论慢性心力衰竭患者自我报告结局和自我管理处于中等水平,但仍有提升空间,提高慢性心力衰竭患者的自我管理水平,有助于改善其自我报告结局。Objective To explore the status quo of patient-reported outcomes and self-management of patients with chronic heart failure(CHF)and their relationship.Methods A total of 314 patients with chronic heart failure and treated at two hospitals in Zhengzhou between January and August 2020 were selected.The patient-reported outcomes instrument for chronic heart failure(CHF-PRO)and the self-management scale of heart failure patients(SMHFP)were used to evaluate the CHF patients.Results The total CHF-PRO score was at a medium level of 72.78±13.09.The standardized score of self-management was 75.36%,which was also in the middle level.The results of correlation analysis showed that the self-reported clinical outcomes were significantly and positively correlated with the scores of all dimensions of self-management(drug r=0.28,diet r=0.11,psychological and social adaptation r=0.30,and symptoms r=0.24)(P<0.01).Conclusion The patient-reported outcomes and self-management of patients with chronic heart failure are at a medium level,but there is still room for improvement.Improving the self-management of patients with chronic heart failure can help to improve their patient-reported outcomes.
