摘 要:目的观察妊娠期糖尿病(gestational diabetes mellitus, GDM)患者血清果糖胺(fructosamine, FMN)水平对新生儿低血糖发生的影响。方法前瞻性选取乌鲁木齐市妇幼保健院2018年1月至2020年1月收治的97例GDM孕妇作为研究对象,收集孕妇基线资料,检测血清FMN水平,按照新生儿低血糖发生情况分为低血糖组和正常血糖组,比较两组孕妇基线资料及血清FMN水平,采用回归分析检验各主要指标与新生儿低血糖发生的关系及对低血糖发生的预测效能。结果 97例GDM孕妇分娩时,有17例新生儿发生低血糖,有80例新生儿血糖水平正常;初步比较低血糖组和正常血糖组基线资料后,经回归分析检验结果显示,GDM患者HbA1c、FMN高水平是新生儿低血糖发生的危险因素(OR>1,P<0.05);绘制受试者工作曲线(ROC)发现,GDM患者HbA1c、FMN水平用于预测新生儿低血糖发生的曲线下面积(AUC)均>0.70,有中等预测效能,且FMN用于预测新生儿低血糖发生的AUC最大。结论新生儿低血糖与GDM孕妇HbA1c、FMN高水平有关,HbA1c、FMN可作为新生儿低血糖的预测指标,且FMN具有更佳预测效能。Objective To observe the effect of serum fructosamine(FMN) level on neonatal hypoglycemia in patients with gestational diabetes mellitus(GDM).Methods 97 pregnant women with GDM admitted to Urumqi Maternal and Child Health Hospital from January 2018 to January 2020 were prospectively selected as the research subjects, baseline data of pregnant women were collected, serum FMN level was detected, the occurrence of neonatal hypoglycemia was recorded, the baseline data and serum FMN levels of pregnant women were compared between the hypoglycemic group and the normoglycemic group, regression analysis was used to test the relationship between the main indicators and the occurrence of neonatal hypoglycemia and the predictive efficacy of hypoglycemia.Results During the delivery of 97 GDM pregnant women, 17 cases of neonatal hypoglycemia occurred, and 80 cases of neonatal hypoglycemia level were normal;after preliminary comparison of baseline data between hypoglycemia group and normoglycemic group, through regression analysis, the results showed that high levels of HbA1c and FMN in GDM patients were risk factors for neonatal hypoglycemia(OR>1,P<0.05);the receiver operating curve(ROC) was drawn and showed that the area under the curve(AUC) of HbA1c and FMN levels used to predict the occurrence of neonatal hypoglycemia in GDM patients were both >0.70,which had moderate predictive efficacy, the AUC of FMN in predicting neonatal hypoglycemia was the largest.Conclusion Neonatal hypoglycemia is associated with high levels of HbA1c and FMN in GDM pregnant women, HbA1c and FMN can be used as predictors of neonatal hypoglycemia, and FMN has better predictive efficacy.
