摘 要:目的探讨^(99)Tc^(m)-MDP SPECT/CT融合显像较常规平面骨显像在钙化防御诊断中的增益价值。资料与方法回顾性分析2019年3月—2020年9月东南大学附属中大医院核医学科42例临床疑诊钙化防御患者,均行全身骨显像和SPECT/CT融合显像,并经皮肤活检确诊。观察全身骨显像和SPECT/CT融合显像(均42例)诊断的真/假阳性和真/假阴性例数,比较两组诊断钙化防御的敏感度、特异度和准确度。结果临床最终确诊钙化防御32例,SPECT/CT融合显像诊断钙化防御的敏感度、特异度及准确度分别为87.5%、60.0%和81.0%;全身骨显像诊断钙化防御的敏感度、特异度及准确度分别为75.0%、50.0%和69.0%。两组的敏感度、特异度及准确度差异均有统计学意义(P<0.05)。结论SPECT/CT融合显像对临床诊断钙化防御较全身骨显像具有较高的增益价值。Purpose To evaluate the value of ^(99)Tc^(m)-methylene diphosphonic acid(^(99)Tc^(m)-MDP)SPECT/CT fusion imaging technology compared with conventional planar bone imaging in diagnosis of calcification defense.Materials and Methods A retrospective analysis was performed on 42 clinically suspected patients with calcification defense in the Department of Nuclear Medicine,Zhongda Hospital affiliated to Southeast University from March 2019 to September 2020.All patients underwent whole-body bone imaging and SPECT/CT fusion imaging,and all were confirmed by skin biopsy.The number of true and false positives and true and false negatives in the whole body bone imaging group and the SPECT/CT fusion imaging group(both 42 cases)were counted,and the differences of sensitivity,specificity and accuracy in the diagnosis of calcification defense between the two groups were further compared.Results Thirty-two cases of calciphylaxis were finally confirmed clinically.The sensitivity,specificity,and accuracy of SPECT/CT fusion imaging group in the diagnosis of calciphylaxis were 87.5%,60.0%,and 81.0%,respectively;whole-body bone imaging group was 75.0%,50.0%,and 69.0%,respectively.There were statistically significant differences in sensitivity,specificity and accuracy between the two groups(P<0.05).Conclusion In the diagnosis of calciphylaxis,SPECT/CT fusion imaging has more preferable diagnostic value than whole body bone imaging.
