摘  要:目的探讨不同金属蛋白酶(MMP)和解聚蛋白样金属蛋白酶(ADAMTS)的mRNA表达与椎间盘退行性变性(简称退变)程度的相关性。方法选取行手术治疗的椎间盘退变患者160例,依据磁共振成像(MRI)检查结果,采用Pfirrmann分级将所有患者分为Ⅰ级组(30例)、Ⅱ级组(32例)、Ⅲ级组(35例)、Ⅳ级组(33例)和Ⅴ级组(30例)。检测所有患者髓核组织中MMP-1 mRNA、MMP-3 mRNA、MMP-9 mRNA、MMP-13 mRNA、ADAMTS-1 mRNA、ADAMTS-4 mRNA、ADAMTS-5 mRNA、白细胞介素(IL)-1αmRNA、IL-1βmRNA、IL-6 mRNA和IL-8 mRNA的相对表达量。采用Spearman相关分析评估各项指标与Pfirrmann分级的相关性。采用Pearson相关分析评估各项指标之间的相关性。结果Ⅳ级组和Ⅴ级组MMP-3 mRNA、MMP-9 mRNA、IL-1αmRNA和IL-1βmRNA相对表达量显著高于Ⅰ级组、Ⅱ级组和Ⅲ级组(P<0.05),且Ⅰ级组、Ⅱ级组、Ⅲ级组依次升高(P<0.05)。Ⅲ级组、Ⅳ级组和Ⅴ级组ADAMTS-4 mRNA和ADAMTS-5 mRNA相对表达量显著高于Ⅰ级组和Ⅱ级组(P<0.05),且Ⅱ级组显著高于Ⅰ级组(P<0.05)。其他各项指标各组间差异均无统计学意义(P>0.05)。Spearman相关分析结果显示,MMP-3 mRNA、MMP-9 mRNA、ADAMTS-4 mRNA、ADAMTS-5 mRNA、IL-1αmRNA及IL-1βmRNA相对表达量与Pfirrmann分级均呈正相关(P<0.05)。Pearson相关分析结果显示,MMP-3 mRNA、MMP-9 mRNA、ADAMTS-4 mRNA、ADAMTS-5 mRNA相对表达量与IL-1αmRNA、IL-1βmRNA相对表达量均呈正相关(P<0.05)。结论MMP-3、MMP-9、ADAMTS-4、ADAMTS-5、IL-1α和IL-1β与椎间盘退变有关,并可反映病情的严重程度。Objective To investigate the correlation between the expression levels of matrix metalloproteinase(MMP)mRNA and a disintegrin and metalloproteinase with thrombospondin motifs(ADAMTS)mRNA with the degree of intervertebral disc degeneration.Methods A total of 160 disc degenerative disease patients were enrolled,and according to the results of magnetic resonance imaging(MRI)and Pfirrmann classification method,they were classified intoⅠlevel(30 cases),Ⅱlevel(32 cases),Ⅲlevel(35 cases),Ⅳlevel(33 cases)andⅤlevel(30 cases).The expressions of MMP-1 mRNA,MMP-3 mRNA,MMP-9 mRNA,MMP-13 mRNA,ADAMTS-1 mRNA,ADAMTS-4 mRNA,ADAMTS-5 mRNA,interleukin(IL)-1αmRNA,IL-1βmRNA,IL-6 mRNA and IL-8 mRNA in the nucleus pulposus were determined.Spearman correlation analysis was used to evaluate the correlation between each indicator and Pfirrmann classification,and Pearson correlation analysis was used to evaluate the correlation between each indicator.Results The relative expressions of MMP-3 mRNA,MMP-9 mRNA,IL-1αmRNA and IL-1βmRNA inⅣandⅤlevel groups were higher than those inⅠ,ⅡandⅢlevel groups(P<0.05),and those inⅠ,ⅡandⅢgroups were increased in turn(P<0.05).The relative expressions of ADAMTS-4 mRNA and ADAMTS-5 mRNA were higher inⅢ,ⅣandⅤlevel groups than those inⅠandⅡlevel groups(P<0.05),and those inⅡlevel group was higher than those inⅠlevel group(P<0.05).There was no statistical significance for the other indicators(P>0.05).Spearman correlation analysis showed that the relative expressions of MMP-3 mRNA,MMP-9 mRNA,ADAMTS-4 mRNA,ADAMTS-5 mRNA,IL-1αmRNA and IL-1βmRNA were positively correlated with Pfirrmann classification(P<0.05).Pearson correlation analysis showed that the relative expressions of MMP-3 mRNA,MMP-9 mRNA,ADAMTS-4 mRNA and ADAMTS-5 mRNA were positively correlated with the relative expressions of IL-1αmRNA and IL-1βmRNA(P<0.05).Conclusions MMP-3,MMP-9,ADAMTS-4,ADAMTS-5,IL-1αand IL-1βare related to the occurrence and development of intervertebral disc degeneration,which can ref
