基于县域医共体模式下新疆地区某县基本公共卫生服务综合评价

摘　　要：目的综合评价新疆某县域医供体模式下的试点县基本公共卫生服务能力,并提出针对性的对策建议。方法 2021年1—3月期间,采用整群抽样方法,对新疆地区某县域医共体试点县10所基层医疗机构进行基本公共卫生服务项目考核,并采用优劣解距离法(technique for older preference by similarity to an ideal solution, TOPSIS)结合秩和比法(rank-sum ratio, RSR)对21项基本公共卫生服务项目进行综合评价。结果 TOPSIS法评价结果显示,H、C、E基层医疗机构排名靠前,C;值分别为0.9907、0.8585和0.8021,与理想值1最为接近;排名靠后的是B、J、I机构,C;值分别为0.3760、0.3327、0.3067,与理想值1相差较大。结合RSR法将评价结果分为3档,H为较好档次,A、B、C、D、E、F、G、J为中等档次,I为较低档次。结论县域医共体模式下新疆地区某县各基层医疗机构基本公共卫生服务项目开展结果区域间和各服务项目间存在差异。TOPSIS法结合RSR法的评价方法,更加准确和全面的评价基层公共卫生服务项目开展情况。Objective To comprehensively evaluate the basic public health service capacity of a pilot county under the county medical community model in Xinjiang, and to put forward targeted countermeasures and suggestions. Methods From January to March 2021, a cluster sampling was used to assess the basic public health service projects of 10 grassroots medical institutions in a county medical community pilot county in Xinjiang, and 21 basic public health service projects were comprehensively evaluated by technique for order preference by similarity to ideal solution(TOPSIS) method and rank-sum ratio(RSR) method. Results The evaluation results of TOPSIS method showed that grassroots medical institutions H, C and E ranked top, with the values of 0.9907, 0.8585 and 0.8021, respectively, which were closest to the ideal value 1. Medical institutions B, J and I ranked lower, with the values of 0.3760, 0.3327 and 0.3067 respectively, which were quite different from the ideal value 1. Combined with RSR method, the evaluation results were divided into three grades. H was the better grade, A, B, C, D, E, F, G and J were the medium grades, and I was the lower grade. Conclusion Under the county medical community mode, there are differences in implementation achievement of the basic public health service projects of the grassroots medical institutions in the county in Xinjiang. TOPSIS method combined with RSR method can more accurately and comprehensively evaluate the development of grassroots public health service projects.
