宫腔镜治疗瘢痕妊娠的研究进展

摘　　要：剖宫产术后瘢痕妊娠指在有过剖腹产或者有过宫腔内手术,再次怀孕后孕囊着床在上次剖腹产的瘢痕处,或者孕囊长在腔内留下的瘢痕处。瘢痕妊娠属于一种罕见异位妊娠,容易引起阴道内出血,严重者将引起子宫破裂,胎盘置入等情况出现,如果不及时处理将严重危及到孕妇及胎儿的生命。剖宫产瘢痕妊娠分为Ⅰ型内源性类型和Ⅱ型外源性类型。目前临床上对于治疗瘢痕妊娠方法较多,包括药物治疗、腹腔镜手术治疗、开腹手术和宫腔镜手术等方法。其中宫腔镜手术治疗和传统手术方法比较,创伤面积小,恢复时间快,成为微创的主要手术方法之一,随着医学的深入研究,对宫腔镜研究也随之增多。本文主要对宫腔镜治疗瘢痕妊娠的研究进展进行综述。Cicatricial pregnancy after cesarean section refers to the implantation of the gestational sac at the scar of the last cesarean section after a second pregnancy,or the scar left by the gestational sac growing in the cavity.Scar pregnancy is a rare ectopic pregnancy,which is easy to cause vaginal bleeding.In severe cases,it will cause uterine rupture,placenta accreta and other conditions.If not treated in time,it will endanger the lives of pregnant women and fetuses.Cesarean scar pregnancy is divided into endogenous type I and exogenous type II.At present,there are many clinical methods for the treatment of cicatricial pregnancy,including drug therapy,laparoscopic surgery,laparotomy and hysteroscopic surgery.Compared with traditional surgical methods,hysteroscopic surgery has become one of the main minimally invasive surgical methods due to its small trauma area and quick recovery time.With the in-depth study of medicine,the study of hysteroscopy has also increased.This paper mainly reviews the research progress of hysteroscopy in the treatment of scar pregnancy.
