摘　　要：目的观察解毒止痒外洗方联合白凡士林软膏治疗角化性湿疹的疗效,分析其症状改善及不良反应发生情况。方法选择120例角化性湿疹患者,采用随机数表法分为观察组与对照组。各60例,对照组采用白凡士林软膏治疗,观察组在对照组基础上给予解毒止痒外洗方治疗,持续治疗2周后,比较2组临床疗效,治疗前后痒程度和皮损程度评分、实验室指标水平[C反应蛋白(CRP)、三酰甘油(TG)、总胆固醇(TC)、空腹血糖(FBG)]及生活质量指标调查表(DLQI)评分,随访1个月后比较复发情况,比较治疗过程中不良反应发生情况。结果观察组总有效率(91.66%,55/60)显著高于对照组(76.66%,46/60)(P<0.05);2组治疗后皮损程度及瘙痒程度评分、DLQI评分、TC、TG、FBG及CRP水平显著下降(P<0.05),且观察组明显低于对照组(P<0.05);观察组复发率明显低于对照组(P<0.05);2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论解毒止痒外洗方联合白凡士林软膏治疗角化性湿疹疗效显著,能够有效改善瘙痒、角化、皲裂等症状,缓解皮损程度,且不易复发,安全性好。Objective To observe the curative effect of Jiedu Zhiyang lotion combined with white vaseline ointment in the treatment of keratotic eczema,and to analyze its symptoms and adverse reactions.Methods A total of 120 patients with keratotic eczema were selected and divided into an observation group and a control group by random number table method,with 60 cases in each group.The control group was treated with white vaseline ointment,and the observation group was treated with Jiedu Zhiyang lotion on the basis of the control group.After continuous treatment for 2 weeks,the clinical effects of the two groups were compared,and the scores of itching degree and skin lesion degree before and after treatment were compared.The levels of laboratory indexes[C-reactive protein (CRP),triacylglycerol (TG),total cholesterol (TC),fasting blood glucose (FBG)]and the scores of Dermatology Life Quality Index (DLQI) were compared.After one month of follow-up,the recurrence and adverse reactions during treatment were compared.Results The total effective rate (91.66%,55/60) of the observation group was significantly higher than that of the control group (76.66%,46/60) (P<0.05).After treatment,the scores of skin lesion and pruritus,DLQI scores,the levels of TC,TG,FBG and CRP in the observation group were significantly lower than those in the control group (P<0.05).The levels of TG,FBG and CRP in both groups were significantly decreased (P<0.05),and the observation group was significantly lower than the control group (P<0.05).The recurrence rate of the observation group was significantly lower than that of the control group (P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).Conclusion Jiedu Zhiyang lotion combined with white vaseline ointment is effective in the treatment of keratotic eczema.It can effectively improve the symptoms of pruritus,keratosis and chapping,alleviate the degree of skin lesions,and is not easy to recur,with a good safety.
