熄风清火活血化痰汤治疗急性缺血性脑卒中风火痰瘀证临床观察

摘　　要：目的观察熄风清火活血化痰汤治疗急性缺血性脑卒中风火痰瘀证的临床疗效及安全性。方法选取2018年1月—2019年12月在淄博市中医医院脑病科住院的40例急性缺血性脑卒中风火痰瘀证患者,按照随机数字表法分为试验组、对照组,各20例。对照组给予西医基础治疗,试验组给予熄风清火活血化痰汤与西医基础治疗。治疗10 d后,观察2组治疗前后的中医证候积分、NIHSS评分、mRS评分、BI评分,血常规、尿常规、肝功能、肾功能、凝血功能的变化。结果治疗后2组中医证候积分、NIHSS评分均低于治疗前(P<0.05),并且试验组低于对照组(P<0.05);2组组内、组间mRS评分、BI评分、血常规、尿常规、肝功能、肾功能、凝血指标比较,差异无统计学意义(P>0.05)。结论熄风清火活血化痰汤可降低急性缺血性脑卒中风火痰瘀证患者的中医证候积分、NIHSS评分,改善神经功能,并且具有良好的安全性。Objective To observe the efficacy and safety of Xifeng Qinghuo Huoxue Huatan decoction in treating acute phase of ischemic stroke with syndrome of endogenous-wind,fire,phlegm and blood stasis.Methods A total of 40 patients with Acute Ischemic Stroke with Syndrome of Endogenous-wind,Fire,Phlegm and Blood Stasis hospitalized in Zibo Hospital of Traditional Chinese Medicine from January 2018 to December 2019 were selected.Patients were randomly divided into treatment group and control group.The control group was given standard western medicine treatment,and the treatment group received standard western medicine treatment and Xifeng Qinghuo Huoxue Huatan decoction.After 10 days of treatment,traditional Chinese medicine(TCM)syndrome score,the NIH stroke scale(NIHSS),modified Rankin scale(mRS),Barthel index(BI)scale,and indicators of blood routine,urine routine,liver function,kidney function and coagulation function were evaluated.Results After treatment,TCM syndrome score and NIHSS score in 2 groups were lower than before treatment(P<0.05),and the experimental group was lower than the control group(P<0.05);There were no significant differences in mRS score,BI score,blood routine,urine routine,liver function,kidney function and coagulation indexes between the two groups(P>0.05).Conclusion Xifeng Qinghuo Huoxue Huatan decoction can reduce TCM syndrome score and the NIHSS score of patients of acute phase of ischemic stroke with syndrome of endogenous-wind,fire,phlegm and blood stasis,improve neurological function,and have good safety.
