摘  要:目的探索亚甲基四氢叶酸还原酶(MTHFR)基因C667T的多态性对Ⅱ、Ⅲ度精索静脉曲张(VC)患者氧化应激损伤的影响。方法选取2019年1月至12月在中国人民解放军北部战区总医院泌尿外科行显微镜下VC手术的62例Ⅱ、Ⅲ度VC患者作为研究对象。依据基因分型分为CC组8例、CT组26例、TT组28例。收集患者的基本信息,检测患者血同型半胱氨酸(Hcy)、精索静脉血氧饱和度、精浆丙二醛(MDA)、精液前向运动(PR)+非前向运动(NP)精子百分率。结果CC组、CT组、TT组的Hcy水平分别为(12.5±1.4)μmol/L、(11.9±2.5)μmol/L、(21.5±4.5)μmol/L,TT组高于CC组及CT组(P<0.05)。CC组、CT组、TT组的精索静脉血氧饱和度分别为(44.5±2.2)%、(38.7±4.0)%、(32.5±4.9)%,TT组低于CT及CC组,且CT组低于CC组(P<0.05)。CC组、CT组、TT组的精浆MDA水平分别为(10.4±2.7)nmol/mL、(14.7±2.9)nmol/mL、(19.4±3.5)nmol/mL,TT组及CT组高于CC组,且TT组高于CT组(P<0.05)。CC组、CT组、TT组的PR+NP精子百分率分别为(42.0±17.8)%、(27.3±19.0)%、(23.5±17.8)%,TT组低于CC组(P<0.05)。结论MTHFR基因C667T的多态性可以增加Ⅱ、Ⅲ度VC患者的氧化应激损伤水平,从而影响患者精液质量。Objective To explore the effect of methylene tetrahydrofolate reductase(MTHFR)gene C667T polymorphism on oxidative stress injury in patients with degreeⅡorⅢvaricocele(VC).Methods 62 patients with VC degreeⅡorⅢwho underwent microscopic VC surgery in Department of Urology of General Hospital of the PLA Northern Theater Command from January to December 2019 were selected as the study subjects.According to genotyping,there were 8 cases in CC group,26 cases in CT group and 28 cases in TT group.The basic information of patients was collected,and the blood homocysteine(Hcy),blood oxygen saturation of spermatic vein,malondialdehyde(MDA)in seminal plasma,and the percentage of forward sperm+non-forward sperm(PR+NP sperm)percentage were measured.Results The Hcy level in CC group,CT group and TT group was(12.5±1.4)μmol/L,(11.9±2.5)μmol/L and(21.5±4.5)μmol/L,respectively.The Hcy level in TT group was significantly higher than that in CC group and CT group.The oxygen saturation of spermatic cord in the CC group,CT group and TT group was(44.5±2.2)%,(38.7±4.0)%and(32.5±4.9)%,re-spectively.The oxygen saturation of the TT group was significantly lower than that of the CT group and the CC group,and that of the CT group was lower than that of the CC group.The MDA levels in the seminal plasma of the CC group,CT group and TT group were(10.4±2.7)nmol/mL,(14.7±2.9)nmol/mL and(19.4±3.5)nmol/mL,respectively.The MDA levels in the TT group and CT group were significantly higher than those in the CC group,and those in the TT group were higher than those in the CT group.The percentage of PR+NP sperm in the CC group,CT group and TT group was(42.0±17.8)%,(27.3±19.0)%and(23.5±17.8)%,respectively.The percentage of PR+NP sperm in the TT group was lower than that in the CC group.Conclusions The polymorphism of MTHFR C667T can increase the level of oxidative stress injury in patients with degreeⅡandⅢVC,thus affecting the semen quality of patients.
